2488616 Primary Facility 2016-05-31 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 11:10 a.m., the cellar was full of water with no bubbling noticed, the (13x9) vent pipe was open and venting to a tank, the (9x5) vent pipe was open and venting to a tank, there was 597psi on the production casing. I took a walk around the location and everything looked OK.

I left the site at 11:40 a.m.

2551676 Primary Facility 2017-01-09 Routine/Complete Inspection No Violations Noted On January 9, 2017 the Department, represented by Frank Hoehle was on site to conduct a Routine Complete Inspection of the Hess R. Pad 1 The permit number associated with this well site is

The wells associated with this pad are as follows: 115-20492 115-20494 115-20727 115-20728

This pad was stable, with no erosion or sedimentation issues at this time. There was excellent vegetation all around the site, and a very small area of pad was left. The surface of the pad remaining has been scarified and is porous, covered with loose gravel.

2852422 Primary Facility 2019-03-12 Routine/Complete Inspection No Violations Noted I performed an inspection of the Hess Pad. The weather was overcast and about 32 degrees F.

The access road was short and solid, with no erosional issues apparent. The road was damp but not snowcovered.

The well pad surface was small and well kept. The gravel on the surface was not compacted and graded well.

The surrounding slopes and land was well vegetated and no areas of erosion were evident. There were filter sock recently filled downslope and a pile of compost was on the pad surface.

The wells were properly labeled.

There were no violations noted.
