2477628 Primary Facility 2016-04-22 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:50 p.m., the cellar was about a quarter full of water with no bubbling noticed, the (13x9) vent pipe was open and venting to a tank, the (9x5) vent pipe was open and venting to a tank, there was 646psi on the production casing. I took a walk around the location and noticed that the perimeter of the site had been worked on and the vegetation was good, everything else looked OK.

I left the site at 13:20 p.m.

2508887 Primary Facility 2016-06-15 Routine/Complete Inspection No Violations Noted Inspection of the Lippincott well pad in conjunction with the Lippincott 1,2,3,4 Pipeline notice of termination inspection. There were no issues observed on the well pad at the time of the inspection.
2528868 Primary Facility 2016-10-17 Site Restoration No Violations Noted Inspection of the Lippencott F P1 Well Pad in response to a submitted Notice of Termination request for the Erosion and Sedimentation Control Permit.

The site has been stabilized and vegetation was found to be greater than 70% uniform perennial growth. Temporary E&S BMPs have been removed from the site. No E&S problems were observed at the time of the inspection. Post Construction Stormwater Management BMPs appear to be installed properly according to the site restoration plans. The well pad is less than or equal to the size indicated on the site restoration plans.

Based on the site inspection, I recommend this notice of termination be APPROVED.

2639149 Primary Facility 2017-08-08 Routine/Complete Inspection No Violations Noted Today,s inspection, 8/8/17 was conducted as a routine inspection. Entire site is stable. No erosional issues observed. No violations noted.
