2284483 Primary Facility 2014-03-06 Routine/Complete Inspection No Violations Noted The Department conducted an inspection on 3/6/14 of the SWN PU DD Neubert – Price Pad (also known as the Bernstein – Price Pad). There are 3 wells permitted for this pad, the Bernstein – Price 1H (115-21303), 2H (115-21271), and 5H (115-20719). The 1H and 2H have been spud. The 1H has been drilled. Stimulation has not occurred yet. The pad construction is complete. Temporary stabilization appears to be adequate. Fill material has been dumped to the right of the access road near the pad entrance. This site should be monitored and further stabilized if necessary. No violations noted.
2288542 Primary Facility 2014-07-21 Routine/Complete Inspection No Violations Noted Flowback procedures were in progress as we arrived at the site. P.J. Winowicz and Geno of Southwestern Energy were at the site during the inspection. The 5H well is in the production phase of operations. According to Mr. Winowicz the 1H and 2H wells are being flowed back. Flowback of the wells commenced on 7/17/14. As flowback fluid flows to the surface of the wells, it is routed through flowback iron, to (2) gas buster tanks. The fluid is then routed via a pump through a trailer containing filter tanks. There are (50) micron filter socks inside the filter tanks used to filter out the solid material in the flowback fluid. Upon filtration of the flowback fluid, the fluid is sent to frac tanks staged behind the trailer. The fluid is transported to a SWN permitted centralized waste impoundment for storage. There are (2) in line heaters, (2) sand traps and (2) separation units used in the flowback process. The entire operation takes place on secondary containment.
2397526 Primary Facility 2015-08-05 Routine/Complete Inspection No Violations Noted On 8/5/15, an inspection was conducted in response to a reported small spill that was called in by SWN on their PU DD Bernstein Price Pad. The spill was identified while crews were in the process of moving the SWN Rig 44 off the pad. As part of the process of breaking down and moving equipment off the pad, small holes were identified in the drilling rig liner that had allowed the soapy water related to power washing the equipment before removal from the containment. Upon arrival at the location, I was directed to the location of the spill. A trench was dug along the edge of the containment. It appears as if this excavation, which was probably initiated in response to the initial spill detection, had caused additional damage to the containment. As evidenced in the photographs, contaminated water was accumulating along this area and was continuing to be released to the pad surface. I contacted Mike Deming by phone and described what I had observed and suggested that they employ a vac truck to suck up the water and cease the release.
2397541 Primary Facility 2015-08-06 Routine/Complete Inspection No Violations Noted As the photographs show, it appears as if the release had increased as contaminated water was still present in the containment in this area, and the amount of “water” present in the excavation had increased. Again, the Department suggested that a vac truck be utilized to help reduce/eliminate the release. Since it appeared as if the release was now potentially reaching a quantity at or above a barrel, Mr. Klemish was told that the Department would now require that at a minimum, biased sampling of the impacted area would need to be conducted. The Department suggested that at this point, SWN test for surfactants, the diesel short list analytes, and metals similar to the Departments 946 suite. Jon Caden, SWN company man on-site thought that by tomorrow, they would be able to pull back some of the containment and would be in a better position to assess the extent of the impact.
2400300 Primary Facility 2015-08-07 Follow-up Inspection No Violations Noted The containment was pulled back a further distance from where it was located during the 8/6/15 inspection in order to determine the presence of staining and potential impact to the pad surface beneath it associated with a spill reported by SWN to the Department on 8/5/15. The liner was pulled back at 8:15 AM and a dump truck arrived at 8:30 AM which was around the time the remediation process of the impacted area of the pad surface associated with the spill reported to the Department commenced on 8/7/15. The pad surface is wet in this particular area. Leidos performed electrical conductivity screening upon completion of the excavation. The highest soil conductivity reading Leidos recorded was (813) microSiemens/cm³ at the bottom of the excavation, EC sample # 4 at N- 41.71926°; W-75.63492°. A total of (20) tons of impacted material, which is equivalent to (1) truckload, has been excavated and sent off site to a landfill for disposal.
