Checked cellar and conductor with meter - no detectable levels of combustible gas detected. Secondary containment in place - have interlocking mats on site. Should be spudding well on 11/9/11.

2451735 Primary Facility 2016-01-21 Routine/Complete Inspection No Violations Noted The Department was on site to inspect the Cabot L. Burts 3 well site (ESX11-115-0074). The wells asociated with this site are as follows:

This site is located in Forest Lake Township, and its last company E&S inspection was on 1-19-2016. The vegetation on site was very well established and there was no erosion or sedimentation issues at this time.

2505618 Primary Facility 2016-07-27 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 10:20a.m., the cellar was full of water with no bubbling noticed, the (13x9) vent pipe was open and venting into a tank, the (9x5) vent pipe was open and venting into a tank, there was 539psi on the production casing, I took a walk around the location and everything looked OK.

I left the site at 10:50a.m.

2599619 Primary Facility 2017-05-31 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:35p.m.

The cellar was full of water with no bubbling noticed. The (13x9) annulus vent pipe was open and venting into a small plastic tank, the (9x5) annulus vent pipe was open and venting into a small plastic tank. There was 483psi on the production casing. I took a walk around the location and there were no obvious concerns noticed. I left the site at 13:00p.m.

The well was in production. There was water and ice 1' below grade in the cellar and there was no bubbling noticed. The (13x9) and (9x5) annuli vent pipes were connected together and going to the same tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2709995 Primary Facility 2018-03-26 Routine/Complete Inspection No Violations Noted I inspected the Burts Well Site on March 26, 2018. The access road was a bit muddy where it passes through a quarry, but was solid enough overall. The pad surface was solid and partly snow covered. The cut and fill slopes were well vegetated, visible through the snow. There were no areas of instability noted. The wells were properly labeled, and the wells on site were as follows:

2805394 Primary Facility 2018-11-20 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water 2' below grade in the cellar with no bubbling noticed and there was no gas detected in the remainder of the cellar. The (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. There was gas detected at the poly vent tank with a MSA Altair 5X multi gas detector that was calibrated on 11/2/18. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
