TIH at time of inspection. Walked site. Several tears observed in secondary containment. Noted them to Company Man. Plastic repair contractor on site already reparing other sections. Company man also indicated they will be restoring secondary containment around new cellars. Will be moving to #5 next. No violations observed at this time.

2435959 Primary Facility 2015-12-28 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 11:00 a.m., the cellar was full of water with no bubbling observed, the (13x9) vent pipe was open and venting to a tank, the (9x5) vent pipe was open and venting to a tank, there was 762psi on the production casing. I took a walk around the location and everything looked OK.
2438954 Primary Facility 2016-01-07 Routine/Complete Inspection No Violations Noted Inspection of the well pad in conjunction with the Corporate Pipeline notice of termination inspection. No issues observed on the well pad.
2496237 Primary Facility 2016-06-24 Site Restoration No Violations Noted Inspection of the Frystak C P1 Well Pad in response to a submitted Notice of Termination request for the Erosion and Sedimentation Control Permit.

The site has been stabilized and vegetation was found to be greater than 70% uniform perennial growth. Temporary E&S BMPs have been removed from the site. No E&S problems were observed at the time of the inspection. The restored pad size is consistent with the PCSM/Site Restoration Plan. Post construction storm water management devices were in place according to approved plans and appear to be functioning properly.

Based on the site inspection, I recommend this notice of termination be APPROVED.

2534022 Primary Facility 2016-11-01 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 13:40p.m., the cellar had water at the bottom with no bubbling noticed. The (13x9) vent pipe was open and venting into a tank, the (9x5) vent pipe was open and venting into a tank. There was 604psi on the production casing. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 14:15p.m.

2560665 Primary Facility 2017-01-27 Incident- Response to Accident or Event No Violations Noted Steve Watson, Environmental Health and Safety Specialist of Cabot Oil and Gas contacted me on 1/27/17 at 10:11 AM to report a spill of antifreeze (glycol) that occurred on 1/27/17 around 9:00 AM. He stated the following during our phone conversation. "An antifreeze line failed on a GasSearch Drilling Services Water Truck picking up production fluid, spilling 5.5 gallons of antifreeze to the pad surface. The hose has been repaired in the truck and the tank has been refilled with antifreeze. The spill happened around 9AM. I was notified at 9:15 AM. Resource Environmental is on the way to assess the spill and screen the impacted area. A backhoe and a roll-off container are either already there or in-route. The spill is considered a small spill. Samples will not be collected. The impacted material will either be transported for disposal to Waste Recovery Solutions in Myerstown, PA or Tunnel Hill Reclamation Landfill in New Lexington, Ohio."

2568324 Primary Facility 2017-02-23 Routine/Complete Inspection No Violations Noted The Department was on site at the C Frystak Well Site to conduct a Routine Complete Inspection. I reviewed the E&S Inspection Log which read that Mike Brilla (TEC) conducted the most recent inspection on 11-29-16 and it was a PCSM Inspection. The site has been restored.
2621667 Primary Facility 2017-08-02 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 11:45a.m.

The cellar was full of water with no bubbling noticed. The (13x9) annulus vent pipe was open and venting into a small plastic tank, the (9x5) annulus vent pipe was open and venting into a small plastic tank. All the annulus vent pipes were connected to one pipe going into the same tank. There was 577psi on the production casing and 479psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 12:30p.m.

2732573 Primary Facility 2018-05-15 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was full of water and there was no bubbling noticed. The (13x9) and (9x5) annuli vent pipes were connected together and venting to the same tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2827623 Primary Facility 2019-01-16 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was full of ice. The (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. There was gas detected at the poly vent tank with a MSA Altair 5X multi gas detector that was calibrated on 1/2/19. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
