Did a 15 bbl top job. 9 5/8" set at 1655' and cemented with 10 bbl cement returns. Drillers log on site and up to date. Secondary containment in place and in good condition. Freeboard in opentops at acceptable levels. No shallow water reported and no significant shallow gas reported. No violations observed.
2343176 Primary Facility 2014-12-17 Routine/Partial Inspection No Violations Noted The Department was on site at the Sadecki well site located at 2899 Franklin Hill Road Hallstead, PA 18822 to conduct a Routine Partial Inspection. The E&S Inspection Log indicated that Casidy Strickland conducted an inspection on 12/12/14 and stated that the site was in good condition. The inspection previous was dated 12/02/14. Operator, please be advised that your inspection frequency does not meet the minimum requirements in 102.4(b)(5)(x). At the time of the inspeciton, the well site was partially snow covered. No obvious E&S issues were found. The site appears to meet temporary stabilization requirements.

2423054 Primary Facility 2015-11-06 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:55, the cellar was full of gravel, the vent pipe was open and set up with a Taylor plug with a 2000psi pressure gauge which had about 70psi on it, there was no gas detected at the perimeter of the cellar, there was 373psi on the production casing and 265psi on the production line. I took a walk around the location, everything looked OK.

2518355 Primary Facility 2016-09-12 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 9:55a.m., the cellar was full of gravel and no gas was detected. The vent pipe was open and set up with a Taylor plug to check for pressure and a 2000psi pressure gauge that was showing about 90psi on it, there was 274psi on the production casing and 176psi on the production line. I took a walk around the location and everything looked OK.

I left the site at 10:20a.m.

2612748 Primary Facility 2017-07-06 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 13:45p.m.

The cellar was full of gravel and there was no gas detected at the perimeter of the cellar. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge and a 2000psi pressure gauge with about 100psi on it. There was 678psi on the production casing and 162psi on the production line. The well was not producing at time of inspection, the main valve going into the GPU was closed. I took a walk around the location and there were no obvious concerns noticed. I left the site at 14:15p.m.

2715004 Primary Facility 2018-04-05 Routine/Complete Inspection No Violations Noted At time of inspection.

The cellar was full of gravel and there was no gas detected at the perimeter of the cellar. The (9x5) annulus was set up with a 2000psi pressure gauge with 115psi on it. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2783684 Primary Facility 2018-10-03 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was temporarily shut in because of a slope failure on the gas line connecting to the well pad. There were representatives from the Department and SWN to assess the situation and plan for repairs. The Sadecki pad is the only one connected to this section of the gas line, no other pads were shut in. The cellar was full of gravel with water on top and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
