2028361 Primary Facility 2011-12-13 Incident- Response to Accident or Event Violation(s) Noted At 10PM on 12/6/11, fifteen (15) gallons of Oil Based Mud was spilled off containment on the surface of the ground on the Well Pad. According to the report, the spill occurred due to operator error as an incorrect button was pushed on the valve controls opening a drain valve, forcing the oil based mud to spill off containment. Absorbent pads were used to contain the spill and the spill was vacuumed up. An excavator was used to remove the contaminated soil where the spill occurred. Location of the spill is the Northeast corner of the Well Pad, in front of where the oil based mud tanks are housed. This spill was not reported to the Department as per the requirements of 25 Pa. Code § 91.33.

2316789 Primary Facility 2014-10-16 Complaint Inspection Pending The Department (Andrea Mullen, Eric Rooney, Jon Sassi, and I) met with Jon Hafer (Chief Oil & Gas) on site at the Squier Unit Well Site to collect soil samples in response to a nearby water quality complaint. The Department collected the below referenced samples to be analyzed with the SAC 961 with the addition of sulfates, lead, calcium, bromide dry weight, and lithium :

2851116 Primary Facility 2019-03-01 Site Restoration No Violations Noted I arrived onsite at 1445 to perform a site restoration inspection in response to an NOT submittal received by the Operator.

After performing an administrative review of the plan and a site inspection, I do not agree that site conditions meet the requirements for the termination of this permit. Namely, several sections of infiltration berm 1 have not attained adequate vegetation and contain exposed erodible material. The rest of the site appears to have attained adequate vegetation.

The Department requests that the Operator inspect infiltration berm 1 for any exposed erodible material and deploy stabilization measures in accordance with your site plan.
