Crews picking up mats and moving rest of equipment off location. Inspected cellar - can hear gas venting through one of the vents. Cupped hand over vent - pressure buildup exists. Can not determine which annulus it is. REquest Operator to identify and report to DEP with findings.

2031194 Primary Facility 2012-01-03 Follow-up Inspection Pending DEP -Steve Watson, Kevin Costello and Andrea Schmid on site 11 am. Meth with Rich Adams and other Chief personnel to investigate backside pressures on well. While on site, Operator installed 100 lb gauge on 9 x 13" annulus, and 100 lb gauge on 9 x 5" annulus. No noticeable increase in pressure on 9 x 13" while on site. Operator going to monitor 9 x 13" for 24 hours to see if any buildup. After 21 mins...SIP on 9 x 5" was 28 psi. Operator will continue venting 9 x 5" to poly tank. Once 24 hr shut in test complete, Operator will report data to DEP. Inspection results Pending at this time.
2310439 Primary Facility 2014-10-02 Drilling/Alteration No Violations Noted
2445793 Primary Facility 2016-01-25 Routine/Complete Inspection No Violations Noted Routine inspection of the Kerr Unit B Well Pad prior to the start of fracking activities of the 2H and 3H wells. I was accompanied by Jon Sassi, DEP Environmental Protection Specialist. During the inspection we met Water Foreman, Ralph Larsen, and Company Man, Bret Fowler, both of Chief. The Department received stimulation notices for both wells on January 22, 2016. At the time of the inspection, fracking continues on the Kerr Unit 4H well.

The site is stable with no observable E & S issues at the time of the inspection. Post Construction Stormwater Management BMPs are installed and appear to functioning properly. Containment was in place around the water and brine tanks, etc, and crews were at work constructing the remaining containment and building the heater houses for the water and brine tanks.

2473919 Primary Facility 2016-03-24 Routine/Complete Inspection No Violations Noted Routine inspection of the Kerr Unit B Well Pad in conjunction with the Notice of Termination inspection of the Kerr B Lateral pipeline. Crews were removing containment and equipment from the recent fracking of the 2H and 3H wells. Much of the surface of the pad was still covered but no issues were observed. The Department will visit the pad at a later date for a more detailed post-frack inspection of the pad.
2715941 Primary Facility 2018-04-06 Site Restoration No Violations Noted This inspection was conducted to confirm Chief's assessment that the project meets the minimum requirements for permit termination. The one infiltration berm for this pad has been installed. The Department’s determination is that the site meets the requirements for permit termination. I recommend the NOT be approved.
