2440245 Primary Facility 2016-01-13 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:50 p.m., the cellar was full of ice and snow, the (13x9) vent pipe was open and venting to a tank, the (9x5) vent pipe was open and venting to a tank, there was 705psi on the production casing. I took a walk around the location and everything looked OK.

I left the site at 13:15 p.m.

2540527 Primary Facility 2016-11-30 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:45p.m., the cellar was about three quarters full of water with no bubbling noticed. The (13x9) vent pipe was open and venting into a small poly tank, the (9x5) vent pipe was open and venting into a small poly tank. There was 555psi on the production casing. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 13:20p.m.

2627713 Primary Facility 2017-08-18 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 14:05p.m.

The cellar was full of water with no bubbling noticed. The (13x9) annulus vent pipe was open and venting into a small plastic tank, the (9x5) annulus vent pipe was open and venting into a small plastic tank. All the annulus vent pipes were connected to one pipe going into the same tank. There was 502psi on the production casing. I took a walk around the location and there were no obvious concerns noticed. I left the site at 14:35p.m.

2714138 Primary Facility 2018-04-03 Routine/Complete Inspection No Violations Noted I inspected the Deriancho well pad for RTNC inspection. The well pad was approximately 80,000 sq. ft. The cut slope and well pad had greater than 70% perennial vegetative cover. There were no temporary BMPs remaining on site. There were two wells associated with this site:

115-20484 115-20590.

There were no erosional issues noted.

There were no violations observed.

2737965 Primary Facility 2018-06-05 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water 1' below grade in the cellar and there was no bubbling noticed. The (13x9) and (9x5) annuli vent pipes were connected together and going to the same tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2833669 Primary Facility 2019-01-30 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was full of snow and ice, I was unable to check for gas. The (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. There was gas detected at the poly vent tank with a MSA Altair 5X multi gas detector that was calibrated on 1/2/19. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
