2282165 Primary Facility 2014-06-23 Incident- Response to Accident or Event No Violations Noted Arrived at the site at 13:05 in response to a (7) gallon spill of a hydrochloric acid. Cabot contacted the Department at 5:30 PM on 6-18-14 to report they lost (7) gallons of hydrochloric acid on the Pad and it has been neutralized and cleaned up. Hydraulic fracturing operations were in progress during the inspection. Halliburton is the subcontractor at the location conducting the operations. A Halliburton truck containing hydrochloric acid, which is used during well stimulation procedures, lost acid out of a hatch on top of the truck while en route to move the truck from near the access road of the Pad to the truck storage area. Apparently, as the truck stopped, acid sloshed back and forth in the truck, eventually escaping either an open, loose or not completely closed hatch on the top of the truck. The acid flowed down the side of the truck to the ground the surface of the Well Pad.
2530968 Primary Facility 2016-10-20 Site Restoration No Violations Noted Inspection of the Lopatofski J P1 in response to a submitted Notice of Termination request for the Erosion and Sedimentation Control Permit. EPS, Jon Sassi, was also present on this inspection.

The site has been stabilized and vegetation was found to be greater than 70% uniform perennial growth. Temporary E&S BMPs have been removed from the site. No E&S problems were observed at the time of the inspection. Post Construction Stormwater Management BMPs appear to be installed properly according to the site restoration plans. The well pad is less than or equal to the size indicated on the site restoration plans.

Based on the site inspection, I recommend this notice of termination be APPROVED.

Jon and I also conducted a general inspection of the pad. Signs and labeling appeared to be correct. The mailbox contained the required items. There were no apparent issues with containment. Conductivity readings of water on the pad were of no concern.

2583323 Primary Facility 2017-04-04 Routine/Complete Inspection No Violations Noted The Department was on site to conduct a Routine Complete Inspection of the well site. The E&S Inspection Log showed that Paul Hoven conducted the final E&S Inspection on 2/14/17, it was a post rain event inspection. The site appears to be stable. Being that the ESCGP for this site has been terminated, please remove the E&S Controls.
