2000264 Primary Facility 2011-08-09 Follow-up Inspection Violation(s) Noted Follow up on report of muc spill. During transport of Frac tank, driver left valve open. Spilled mud from pad onto Rte 106 and onto Hwy 81 north to approx. mile marker 212.7. Less than a barrel observed on pad. Followed dust trail up highway and could see where truck pulled over. At pulloff, was a small pool of mud off berm of highway (freshwater -based). REcommend operator clean up mud and discuss issue with drivers. Operator shall clean up mud from pad and also from side of Hwy 81. Upon completion, operator to report to DEP for follow up. 20 " set at 70'. 13 3/8" set at 650' with 30 bbl returns, 9 5/8" set at 1623' with 37 bbls returns. Hole shut in until big rig is moved on site to drill out to KOP and lateral.
