Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
1974888 Primary Facility 2011-05-11 Drilling/Alteration No Violations Noted Depth of drilling = 431'. Surface casing (13 3/8") set at 431'. Set with 383 sks of 15.2 lb cement with 1.36 Y. Reported 40 bbls return. 20" conductor set at 140'. Bumped plug at 21:15 on 5/10/11. Walked site with Rig mgr. David Phillips. Discussed fixing and maintaining secondary containment. Commented on flare stack height. Height was approximately 8' high which appeared to be low and potentially unsafe. Looked at area of previous spill near sand trap under shakers. Appears spill has been adequately cleaned at this time and this violation has been corrected. Fill slope also stabilized. Inspected cellar for bubbling - none observed.
1987226 Primary Facility 2011-07-07 Follow-up Inspection Pending Drilling complete. Inspected cellar. Fluid above 9 5/8" & conductor. Observed steady stream of bubbling along inside wall of conductor on eastern side. Cannot see source of bubbling due to level of fluid. Further investigation required. Gas well completions report ultimately reviewed 3/2/12. Integrity evaluation of gas well done during GMI investigation found no viols. GMI closed 4/24/13 via non-impact determ ltr.
1987346 Primary Facility 2011-07-07 Routine/Complete Inspection No Violations Noted
1989187 Primary Facility 2011-07-20 Follow-up Inspection Violation(s) Noted Follow up on evidence of bubbling around well head. Observed bubbling in between 9 5/8" and 13 3/8" casing. Minor bubbling outside of 13 3/8" casing. Tagged TOC in 9 5/8 annulus at @ 3' below top of casing. TOP cement tagged at @ 12' below casing on 13 3/8" annulus. Violations for defective cement and no well tag cited. Gas well completions report ultimately reviewed 3/2/12. Integrity evaluation of gas well done during GMI investigation found no viols. GMI closed 4/24/13 via non-impact determ ltr.

2053521 Primary Facility 2012-03-20 Routine/Complete Inspection Pending Inspected current status and condition of well. Well has been completed and fracced. Well is not online. Observed minor bubbling on 9 x 13 3/8" annulus at the surface. No other bubbling oberved at the time of this inspection. A Mercer service rig is on site and a small crew of workers. DEP will follow up with Operator on condition of well. Gas well completions report ultimately reviewed 3/2/12. Integrity evaluation of gas well done during GMI investigation found no viols. GMI closed 4/24/13 via non-impact determ ltr.
2066945 Primary Facility 2012-05-10 Drilling/Alteration No Violations Noted At time of inspection- Service Rig set up on 1H - preparing wells for production. Inspected cellar - intermittent bubbling observed on outside of 9 5/8" casing. (minor bubbling about every second). Well is shut in - not online yet. Pipleine crews working nearby at time of inspection.
2278322 Primary Facility 2014-06-12 Site Restoration Pending Well Site Restoration Report (8000-PM-OOGM0075) submitted with missing information.

2434497 Primary Facility 2015-12-14 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 11:50 a.m., the cellar was full of gravel, the vent pipe was open and set up with a Taylor plug, there was no gas detected at the perimeter of the cellar, there was 358psi on the production casing and 287psi on the production line. I took a walk around the location and everything looked OK.
2508878 Primary Facility 2016-05-24 Routine/Complete Inspection No Violations Noted Inspection of the Hollenbeck well pad in conjunction with the Snake Creek Gathering Pipeline notice of termination inspection. There were no issues observed on the well pad at the time of the inspection.
2528988 Primary Facility 2016-10-18 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 13:35p.m., the cellar was full of gravel and there was no gas detected at the perimeter of the cellar. The vent pipe was open and set up with a Taylor plug to check for pressure, the valve on the vent line going to the tank was closed. There was 285psi on the production casing and 277psi on the production line. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 14:15p.m.

2619622 Primary Facility 2017-07-25 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 14:55p.m.

The cellar was full of gravel and there was no gas detected at the perimeter of the cellar. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge, the valve going to the production tank was closed. There was 410psi on the production casing and 210psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 15:35p.m.

2726965 Primary Facility 2018-04-27 Routine/Complete Inspection No Violations Noted At time of inspection.
