Pumped 6 bbls into formation and pressure rose to 1500#. Shut down for 5 mins - had no bleed off. Pressure up to 2000# and shut down - no bleed off. Pressured back down to 1500# and shut well in for 24hrs. On 5/13 - came back and had pressure build up of 700#.

2070096 Primary Facility 2012-05-22 Drilling/Alteration No Violations Noted Follow up on remedial work. Service rig on well. Since last inspection = have swabbed well dry to 5800'. Perforated 5700- 4holes. Pumped 2bbl freshwater, pumped 55 sks (Standard A/.2%CD20/ .5% Gasblock) Pressured up to 750#. Pumped 4.75 bbl into perforations. Pressured increased to 1500#, then increased pressure to 2000# and held at this level for 10 min with no reported bleed off. The plugs were then drilled out, and the hole was cleaned out to CIBP set at 6800'. Ran noise/temp log. Logs didnt seem to show any improvement as of this morning. See inspection record 2070092 for plan for today on this well. No violations observed.
2074427 Primary Facility 2012-05-30 Drilling/Alteration No Violations Noted Follow uo on remedial work. As of 5/22 - pumped 44.5 bbl lead, pumped 11.75 bbls cement (15.5 #) loaded hole with 86 bbls. Caught pressure - eased into throttle @ 2 bpm. pressure increased to 800# and began to ease up @ 1.3bpm. Pumped a total of 96.3 bbl flush and pressure @ 2000#. Got 2.3 bbls squeezed away - not sure if went into formation or not. Drilled out and swabbed hole dry. Cement was right below perf's (5700 - 5702). Put meter on but meter did not work over weekend. Have installed new meter on this morning and will monitor pressures/flow. No violations observed.
2085103 Primary Facility 2012-07-16 Drilling/Alteration No Violations Noted SErvice Rig on well. Currently still getting influx of gas from open hole into 5 1/2" (cemented to surface). Venting upper Marcellus to flowback tank - have @ 3lbs on tubing. Still gaining some pressure on 5 1/2". As of this morning had 50 psi on 5 1/2" after 30 mins. Will continue to monitor flow and pressures at this time. No violations observed.
2085263 Primary Facility 2012-07-20 Drilling/Alteration No Violations Noted Follow up on remedial work. Met Company Man Larry Arnold on site. The most recent cement squeeze was 87 bbls @ 7020'. At @ 4550lbs were able to pump at rate of 1.5 bbl/min. CIBP is set at 7250'. According to daily reports - pre-squeeze pressure on 5 1/2" was up to 500lbs. Post-squeeze conditions are currently showing very little gas. Mr. Arlnold installed a 100lb ga. on 5 1/2 while I was present. the gauge showed no pressure increase. The needle vavle was left open. After @ 1.5 mins, the audible gas venting was no longer heard. A cup of water was then held over needle valve. No bubbling was observed in the water indicating no gas flow at that time. At this time the cement and CIBP have not been drilled out. Operations shut down for weekend and will continue to monitor pressures on well. Cabot also collecting water samples from residences in the area of this site. DEP offsite at 15:30. No violations observed.
2086058 Primary Facility 2012-07-24 Drilling/Alteration No Violations Noted Workover rig on site. On 7/21/12 pressure on 5 1/2" was 95 lbs (6 am). Well was bled off. At 16:00 that afternoon pressure was back up to 80 lbs. On Monday 7/23 at 6 am, pressure was up to 365lbs. Well was bled down. Then tripped in hole with tubing and bit. On 7/24 spotted acid @ 5700' in location of perforations previously made. Ran injection test and were able to pump 50 bbls @ 2 bpm with 2660lbs. At time of visit, crew in process of setting cement retainer @ 5680'. On 7/25, 400 sks of cement are being delivered to the site to perform squeeze @ 5700'. The pressure gauge on the Stalter 1H read 260 lbs which is up from 120lbs indicated on 7/20. On Stalter 2H found lower master valve was shut. When opened the pressure was 675lbs on the 5 1/2". Well was bled down and flowed back 6 bbls of fluid at that time. As of 6:00 am this morning (7/24) pressure was up to 780 lbs on the Stalter 2H - 5 1/2" casing. DEP will continue to follow up on remedial efforts.
2089480 Primary Facility 2012-08-01 Drilling/Alteration No Violations Noted Service rig on well. Tubing is currently out of hole. Will be flow testing 5 1/2" later today when opacker arrives. Last perforation 2486-2488'. Cement retainer set at 5685'. Pressures as of this morning= 1H is 475psi, 2H is 810 psi. Have flowmeter currently on 8V 5 1/2" and venting to tank. No violations observed at this time.

Pump truck on site to suck out fluid from cellar inorder to identify and label all vent lines for future reference. All vents are now labeled with paint pen. (20 x 13 3/8"), (13 3/8" x 9 5/8"), (9 5/8" x 5 1/2"), and (5 1/2").

The Department finds that the site is permanently stabilized, and that the record drawings dated 10/15/2010 match the as-built drawings which reflect conditions on the ground as observed by the Department. 3 wells have been drilled on this pad, the 1H (115-20517) and 2H (115-20496) both appear to be in production, while the 8V (115-20457) is not tied into production. There are 3 other wells associated with this well pad, the 3H (115-20506), the 4H (115-20497, and the 6H (115-20522), none of which have been drilled. I recommend this notice of termination be approved.
