2032291 Primary Facility 2012-01-06 Incident- Response to Accident or Event Violation(s) Noted NERO EC&B Act 2 ROL granted on 4/5/12, MBC data cleaan-up 5/6/15. Arrived at the site at 10AM in response to a production fluid spill that occurred on January 5, 2012 at 3:30 PM. A hammer union came loose on the production fluid line associated with the Cosner 5H separation unit causing the production fluid to leak and contact the surface of the ground on the Well Pad. The estimated quantity of the release was 2-3 bbls. Cleanup procedures started last night, 1-5-12. More cleanup will be required as there is still evidence of the production fluid on the surface of the ground on the Well Pad in a close proximity to where the Cosner 5H separation unit is housed.
2053580 Primary Facility 2012-01-10 Follow-up Inspection Outstanding Violations - No Viols Req'd GDS was in the process of excavating test pits in preparation for the URS sampling event. As of 11 AM (5) test pits have been dug and sampling just got underway in a test pit 30-40 feet away from the Cosner 5H separation unit, where the hammer union leaked production fluid. The test pits have been dug to a depth of 2 -3 feet. The representative from URS sampled from the base of the test pit.
2060083 Primary Facility 2011-12-22 Follow-up Inspection No Violations Noted NERO EC&B Act 2 ROL graanted 1/26/15, MBC daataa cleaan-up 5/6/15. Folow up on report of incident associated with Production unit malfunction associated with 5H well. Reportedly a coil ruptured on the production unit and a small release of fluid was observed. At the time of inspection I spoke with the production crew who was on site installing the new unit. According to the crew on location, a surge of sand likely caused the coil to break down and eventually wore a hole through the steel elbow and ultimately causing fluid to escape. A minimal amount of spray (less than 20 gallons) ws reported. The flow was immediately shut off according to Cabot. When i arrived on site, the damaged production unit was already loaded onto a truck and the new unit was in place. Based on my inspection, there was no evidence of any significant impact to the pad from the reported release.

2720920 Primary Facility 2018-04-18 Routine/Complete Inspection No Violations Noted Arrived on location at: 11:30. Weather: Clear, cool, ~ 40 degs. (4) wells were present and in production, the Cosner 4H, 5H, 2H & 3H. All wells (13 3/8"x 9 5/8") and (9 5/8"x 5 1/2") annuli were open and venting, plumbed to a common, shared vent tank. A Gas check performed at shared vent tank showed 100% LEL, 100% CH4. *Due to the possible commingling of gas within the (13x9) and (9x5) annular vents to shared vent tank, a follow-up inspection will need to be scheduled to check the (13x9) cemented annuli for any sustained presence of gas. Please contact me when these (13x9) vents can be isolated and rechecked. (kenkennedy@pa.gov).

- All wells Prod. casing, Static and Differential pressures were being monitored by wireless sending units sending data to onsite Total Flow Control Unit "TFCU". Well pressures were taken off of TFCU readouts beside each wells gas production units "GPUs". 2725023 Primary Facility 2018-04-30 Routine/Complete Inspection No Violations Noted Arrived on location at: 08:45. Weather: Clear, cool, ~ 40 degs. (4) wells were present and in production, the Cosner 4H, 5H, 2H & 3H. Onsite in follow-up to recheck the isolated, venting (13 3/8"x 9 5/8") cemented annuli as requested in initial insp. report of 4/18/18. Upon arrival I met with Cabot O&G Rep., Aaron Onks. This morning Aaron had isolated all (4) wells (13 3/8"x 9 5/8") annuli and vented to atmosphere at ~ 08:00. Gas checks were performed at all (4) wells (13 3/8"x 9 5/8") annuli using a ALTAIR 5X gas meter, calibrated on 4/26/18. No gas was detected at any of the wells (13 3/8"x 9 5/8") cemented annuli. It appears that the initial gas detected at the shared poly vent tank on 4/18/18 was from the (9 5/8"x 5 1/2") annuli vents. Being that all (4) wells 5 1/2" casing cement was left down, beneath the intermediate casing shoe, this venting is expected and not a violation.
