I took a walk around the location and everything looked OK.
2537819 Primary Facility 2016-11-17 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 10:00a.m., the cellar was full of water with no bubbling noticed. The vent pipe was open and set up with a Taylor plug to check for pressure. I couldn’t get any pressure reading because the transducers were wrapped in plastic, it appears the well is going to be painted. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 10:40a.m.

2548894 Primary Facility 2016-12-12 Routine/Complete Inspection No Violations Noted The Department, represented by Frank Hoehle was on site to conduct a Routine Complete inspection of the Holbrook well site. (ESX This well site was formerly owned by WPX.

There are two wells associated with this site: 1V 115-20099 (inactive as of 6-24-16) 2H 115-20379

This site had excellent vegetation, even visible through snow cover. There were woody plants established on the fill and cut slopes and the grasses were well established all over the site as well. There were no erosional issues with the site, all slopes were stable and there were no failures of BMPs to note.

2626887 Primary Facility 2017-08-15 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 13:55p.m.

The cellar was set up with a plastic corrugated pipe around the well for inspection access; there was water above the surface casing with no bubbling noticed. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge. There was 178psi on the production casing and 111psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 14:35p.m.

2736328 Primary Facility 2018-05-31 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was set up with a corrugated pipe around the well for inspection access. The (20x13) and (13x9) annuli were below the water level and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2794005 Primary Facility 2018-10-24 Routine/Complete Inspection No Violations Noted At time of inspection.

Met with Cody Stanton (Production Foreman) for SWN. The well was in production. The cellar was set up with a plastic corrugated pipe around the well for inspection access. The cellar was full of water and there was no bubbling noticed. Cody used a Crystal pressure gauge to get the pressures for the (9x5) annulus, production casing and tubing. Cody bled the pressure down on the (9x5) annulus from 435psi to 345psi. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
