2060163 Primary Facility 2012-04-17 Drilling/Alteration No Violations Noted Patterson 252. Co. Man provided cement reports for 13 3/8", 9 5/8" and 7" casing strings. drilling ahead at 6537' on SOBM. All cement reports indicate adequate returns to surface with no fall back reported. Drilers log on site and up to date. No groundwater reported and no shallow gas shows detected above background levels. Secondary containment in place and in good condition. Last well on pad at this time. Rig will move to next location upon completion of this well.

2487303 Primary Facility 2016-05-24 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 11:00 a.m., the cellar was full of water and overflowing onto the pad with no bubbling noticed, the (13x9) vent pipe was open and venting to a tank, the (9x7) vent pipe was open and venting to a tank, there was 572psi on the production casing. I took a walk around the location and everything looked OK.

I contacted Steve Watson of Cabot Oil & Gas to let him know about the cellar and suggest it be vacced out. I left the site at 11:35 a.m.

2490581 Primary Facility 2016-05-31 Site Restoration No Violations Noted Inspection of the Warriner R P4 Well Pad in response to a submitted Notice of Termination request for the Erosion and Sedimentation Control Permit.

The site has been stabilized and vegetation was found to be greater than 70% uniform perennial growth. Temporary E&S BMPs have been removed from the site. No E&S problems were observed at the time of the inspection. The restored pad size is consistent with the PCSM/Site Restoration Plan. Post construction storm water management devices were in place according to approved plans and appear to be functioning properly.

Based on the site inspection, I recommend this notice of termination be APPROVED.

2544179 Primary Facility 2016-12-09 Incident- Response to Accident or Event No Violations Noted Steve Watson, Environmental Health and Safety Specialist of Cabot Oil and Gas contacted me on 12/8/16 at 4:17 PM to report a spill of production fluid that occurred on 12/8/16 at 3:30 PM. He stated the following during our phone conversation. "A well tender was performing routine maintenance on the well. He was adding soap sticks into the well to flow the wells back. He opened a valve on the well to dump production fluid into a production fluid tank. The fluid went into the tank and overflowed out of a hatch on the top of the tank onto the pad surface. The tank is a double walled tank with no containment around it on the pad surface. The spill volume is unknown at this point, but more than (5) gallons of the fluid were spilled to the pad surface, so the spill is considered a reportable volume. There is a (30) foot area of staining on the pad surface near the tank. We will cover the spill area until Resource Environmental gets here at 8AM tomorrow, 12/9/16, to delineate the extent of the contaminatio
2545543 Primary Facility 2016-12-19 Administrative/File Review No Violations Noted This administrative inspection has been created to document a production fill spill in the range of (10-20) gallons to the pad surface that occurred on 12-8-16 as documented in the Department's 12-9-16 inspection report. Cabot submitted a Remediation Summary Report prepared by Resource Environmental of Montrose, PA to the Department on 12-15-16. The report contains a site map of the release area and perimeter of excavation, (5) test pit locations dug for soil conductivity screening, conductivity readings measured after the final excavation and a remediation summary report containing a description of the incident, remedial action and waste disposal information. The description of the incident in the report states that the following. The spill of (10-20) gallons occurred on 12-8-16. An unanticipated surge of production fluid flowed through the dump line to the production fluid tanks when a well tender was performing maintenance. The spill impacted the immediate area of the production fluid tanks. The re
2660797 Primary Facility 2017-11-15 Routine/Complete Inspection No Violations Noted Arrived on location at: 13:30. Weather: Clear, mild ~ 44 degs. (2) wells have been drilled on this pad, the Warriner, R. 4H and 8H. Both wells were in production at time of inspection. Both wells (13x9) and (9x5) annuli were open and venting, plumbed to a common vent tank. The (5/P) annular vent was shut in to SCADA pressure monitor sending signal to readout mounted by GPU. A Gas check performed at the common annular vent tank showed 5% LEL, 0% CH4. -All wells Prod. casing, Static and Differential pressures were being monitored by wireless sending units to onsite Total Flow Control Unit "TFCU". (See individual well insp. reports for pressures.) - TFCU readings were taken off LED readouts mounted by GPUs at ~ 14:00. (SP= static press., CP= prod. casing press., DP= differential press.) - 8H: SP= 444 psi, CP= 430 psi., DP= 266 psi. - Overall, wells and pad were found in good condition. No obvious concerns noted at time of inspection. No violations noted. Left location at 14:30.
