The site has been stabilized and vegetation was found to be greater than 70% uniform perennial growth. Temporary E&S BMPs have been removed from the site. No E&S problems were observed at the time of the inspection. The restored pad size is consistent with the PCSM/Site Restoration Plan. Post construction storm water management devices were in place according to approved plans and appear to be functioning properly. Based on the site inspection, I recommend this notice of termination be APPROVED. |

He stated the following during our phone conversation. "A well tender was performing routine maintenance on the well. He was adding soap sticks into the well to flow the wells back. He opened a valve on the well to dump production fluid into a production fluid tank. The fluid went into the tank and overflowed out of a hatch on the top of the tank onto the pad surface. The tank is a double walled tank with no containment around it on the pad surface. The spill volume is unknown at this point, but more than (5) gallons of the fluid were spilled to the pad surface, so the spill is considered a reportable volume. There is a (30) foot area of staining on the pad surface near the tank. We will cover the spill area until Resource Environmental gets here at 8AM tomorrow, 12/9/16, to delineate the extent of the contaminatio |

(2) wells have been drilled on this pad, the Warriner, R. 4H and 8H. Both wells were in production at time of inspection. Both wells (13x9) and (9x5) annuli were open and venting, plumbed to a common vent tank. The (5/P) annular vent was shut in to SCADA pressure monitor sending signal to readout mounted by GPU. A Gas check performed at the common annular vent tank showed 5% LEL, 0% CH4. -All wells Prod. casing, Static and Differential pressures were being monitored by wireless sending units to onsite Total Flow Control Unit "TFCU". (See individual well insp. reports for pressures.) - TFCU readings were taken off LED readouts mounted by GPUs at ~ 14:00. (SP= static press., CP= prod. casing press., DP= differential press.) - 8H: SP= 444 psi, CP= 430 psi., DP= 266 psi. - Overall, wells and pad were found in good condition.
