Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
1883136 Primary Facility 2010-05-05 Drilling/Alteration No Violations Noted Jay lewis- Co. man on site. HAve conductor set at 30' (20"). Currently running 13 3/8" water string in hole to a depth of 433' which will be cemented to surface with 110% excess. I recommended to Co. Man to install secondary containment under compressors and other equipment on site where potential leaks may occur. Drillers log was on site and filled out up to date.
1903238 Primary Facility 2010-07-29 Routine/Complete Inspection No Violations Noted
2034619 Primary Facility 2012-01-09 Routine/Complete Inspection No Violations Noted DEP on site 13:00. No activity on site. Routine inspection. Well is online. Cellar appears partially cleaned out. Bottom of cellar is filled with gravel. No standing water present. Gauge on production string reading @3000psi. no other gauges present. Have vent line open to condensate tanks. Request Operator to identify what vent is venting and set up shcedule to do shit in test and sample. If this has been completed already, please provide DEP with data.
2051302 Primary Facility 2012-03-14 Follow-up Inspection No Violations Noted DEP - Steve Watson and Eric Rooney on site at 10:15. Met Ralph Larsen of Chief to inspect cellar as part of potential gas migration investigation. Chief removed cement from cellar to expose tops of conductor and surface water string. Filled cellar with water to just above the tops of casings and observed for bubbling. Waited approximately 15 mins. There was no presence of bubbling during the inspection. Pump truck removed water from cellar. DEP offsite @ 10:45. Well is online.

2441776 Primary Facility 2016-01-19 Routine/Complete Inspection No Violations Noted Inspection of the Stone Unit Pad for current or potential E&S issues due to a recent drilling permit application for the #3H well. The site is less than 5 acres. The site is stable, vegetation coverage around the pad and access is stable, and no erosion issues were observed. The PCMS controls appear to be installed according to the 2010 E&S and restoration plans. They basically consist of a drainage channel around the pad that drains on both sides to a sediment basin and appear to be functioning properly. The access road is in good condition and the PCSMs (drainage channel and culverts, and etc) are in place according to the plans referenced above with no apparent problems.

Shortly after our arrival at 10:30, wet met with Gilbert Blevins, site company man, consultant of Chief. Mr. Blevins stated the following information. "We just finished drilling out frac plugs on the 3H well. We started to drill out the plugs yesterday. There were (12) frac plugs drilled out. The plugs were drilled out at a rate of (15) barrels an hour, (3) barrels in 3.25 hours. The Rain for Rent frac tanks contain water from the frac plug drill out process and water used during the frac. The frac job was completed last week. All of the wells are currently shut in."

Chief submitted a stimulation notification to the Department on 5-18-16. The notification indicated stimulation of the Stone Unit 3H would commence on 5/24/16 at 6:00 AM. According to our records, there are (8) wells drilled on the pad. The 2H, 1V, 5H and 4H wells were in production. The 6H, 7H and 8H have b
