1847922 Primary Facility 2009-12-03 Drilling/Alteration No Violations Noted At time of the inspection, Site ID sign and E/S Plan were on site. E/S controls were in place and appeared to be working properly. According to the Company Man on site Ziegenfuss was on site drilling the conductor on November 27th. The conductor was set at 80'. Rig GDS 9 was being set up during inspection and drilling was anticipated to resume later the same day. The areas underneath the rig, associated tanks, and other equipment has been lined with plastic and bermed.
1849762 Primary Facility 2009-12-10 Drilling/Alteration No Violations Noted At the time of inspection I met with Company Man Robert Hurst. Drilling rig on site is GDS Rig #11. Operator was drilling a 6-1/4¿ vertical hole on air down to a proposed depth of 7090¿. Once the vertical portion is complete, the hole will be loaded with mud. The Company man indicated a larger rig will be brought in to drill the lateral portion of the well. Kickoff point is approximately 6284¿. The company man indicated the surface casing was drilled to 1507¿. The pipe was cemented with 530 sks of Class A cement and had 15 bbls return to pit. Cement report indicated that floats held.

2437933 Primary Facility 2016-01-05 Site Restoration No Violations Noted Inspection of the Larue C P1 Well Pad in response to a submitted Notice of Termination request for the Erosion and Sedimentation Control Permit.

The site has been stabilized and vegetation was found to be greater than 70% uniform perennial growth. Temporary E&S BMPs have been removed from the site. No E&S problems were observed at the time of the inspection. The restored footprint of the pad is less than, or equal to, the dimensions indicated in the plan. Post construction stormwater management devices were in place according to approved plans and appear to be functioning properly.

There is a soil stockpile located just off the northeast corner of the pad that is properly stabilized but it is not indicated on the PCSM/Site Restoration Plan.

Based on the site inspection, I recommend this notice of termination be APPROVED.

2437947 Primary Facility 2015-12-22 Site Restoration No Violations Noted Inspection of the Larue C P1 Well Pad in response to a submitted Notice of Termination request for the Erosion and Sedimentation Control Permit.
2493900 Primary Facility 2016-06-14 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 11:05a.m., the cellar was empty with no gas detected at the perimeter of the cellar, the (9x7) vent pipe was open and venting to a tank, there was 492psi on the production casing. I took a walk around the location and everything looked OK.

I left the site at 11:40a.m.

2816235 Primary Facility 2018-12-13 Routine/Complete Inspection No Violations Noted I performed a RTNC inspection of the LaRue well pad in Dimock, PA. The weather at the time of inspection was flurries, 31 degrees F. There were small patches of light snow scattered around. The ground was frozen and solid.
    The well pad itself was small and the cut slope and fill slope were all well vegetated and solid.  The vegetation on the slopes was sufficient for stabilization and there were no erosional difficulties noted.
    The access road was short and solid with no ruts.  
    The wells on site were all properly labeled and the produced water tanks were in a galvanized steel containment.
    There were no violations noted
