The inspection was conducted to confirm COG’s assessment that the site meets the minimum requirements as defined in Chapter 102.22(C) for permanent stabilization which would thus fulfill the physical requirements necessary for the permits termination. The Department’s determination is that the entire site is in fact permanently stabilized and meets the criteria necessary for the permit’s termination. I recommend this notice of termination be approved.
2490191 Primary Facility 2016-06-03 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 9:00 a.m., the cellar was empty with cement between the (20x9) annulus with no gas detected, the (9x4) vent pipe was open and venting to a tank, there was 495 psi on the production casing. I took a walk around the location and everything looked OK.

I left the site at 9:45 a.m.
