At the time of inspection, there was no activity on site. Gas seepage was observed in the cellar of the well. Bubbling was observed coming to the surface around the 9-5/8 casing. Due to the amount of water in the cellar, it could not be determined a the time of inspection where the gas seepage was coming from.

The inspection was completed as a follow up of a previous inspection performed on 9/25/09 where gas seepage was observed in the cellar of the well. During this inspection, bubbling was observed coming to the surface around the 16" conductor. There was several feet of water in the cellar of the well. Due to the amount of water in the cellar, it could not be determined where the gas seepage was coming from.

1876537 Primary Facility 2010-04-13 Follow-up Inspection No Violations Noted I inspected the well head for evidence of leakage. There was standing water present in the cellar. Visible bubbling was observed along the outside of conductor. The 9-5/8" vent was open. The well was shut in and taken offline. A new well head has been placed on the well. Gas readings were collected using a Model 60 gas meter. A reading of 38% Methane was detected when the gas probe was inserted into the 9-5/8" vent pipe riser. No other detections of were observed around the well head. There was no activity on location. The reserve pit is still open containing residual drill cuttings and fluid. There was approximately 6' of freeboard in the pit.

Set plug and pressured up to 1000lbs which held for 30 mins with no leak off. Perforated casing 860-862' with 4shots/ft for total of 8 holes using a 3 3/8" Tag Gun. Pressured up on perforated area to 1000lbs for 30 mins and had no leak off. Cement retainer set at 850'. TOH and shut in well.

Currently there is no activity on this well. There is a 300 lb gauge on the 13 3/8" casing which continues to show 0 pressure. There was apporximately 6" of standing water in the cellar. Intermittent bubbling was observed coming to surface around the outside of the conductor.
1883297 Primary Facility 2010-05-10 Plugging(Includes Plugged/Mined Through) No Violations Noted At the time of inspection I met with Mark Howard of Boots and Coots on site. The well is shut in. Currently there is no activity on this well. There was approximately 1-2" of standing water in the cellar. Minor bubbling was observed coming to surface around the outside of the conductor. The bubbling was continuous.
1886414 Primary Facility 2010-05-24 Plugging(Includes Plugged/Mined Through) No Violations Noted At the time of inspection I met with Mark Howard of Boots and Coots who has been overseeing plugging activities for Cabot. Mr. Howard summarized the recent activities. The 7" and 9 5/8" casings were mechanically cut at 300'. An injection rate could not be established at this depth, and therefore no cement was squeezed into the formation. Cement was spotted at 300', and top of cement was tagged at 200' in the 7" casing. The top of the 7" casing was cut approximately 6' below the top of the 9 5/8" casing. The cut section of 7" casing (@ 11') was pulled. The remaining open sections of casings were then filled to surface with gravel. A metal plate was welded on top of the 9 5/8" casing with a 2" port. A 2" riser with a bull plug and needle valve was screwed into the metal plate. A section of 4 1/2" casing was welded to the well extending approximately 4' above surface. A metal plate containing the well permit number was welded to the top of the 4 1/2" riser.
1886583 Primary Facility 2010-05-18 Plugging(Includes Plugged/Mined Through) No Violations Noted Crew trying to pull section of 7" casing cut at 425'. A mechanical cut was made on the 7" and 9 5/8" casings at 425'. Crew could not pull the 7" out of the hole while I was on site. Plan is to TIH with mechanical cutter and re-cut casing.
1887834 Primary Facility 2010-05-27 Plugging(Includes Plugged/Mined Through) No Violations Noted At the time of inspection, there was no activity on site. All drilling equipment has been mobilized off location. The cellar of the well had approximately 2¿ of standing water in the western portion of the hole. Minor ¿constant bubbling was observed in between the 13 3/8¿ conductor and 9 5/8¿ casing. The bubbling appeared on the eastern side of the well head in an area where the 9 5/8¿ butted up against the 13 3/8¿ conductor. A 100 lb pressure gauge installed on the 2¿ riser connected to the 9 5/8¿ casing showed 0 psi at the time of inspection. DEP collected a video of the conditions of the wellhead during this inspection.
1887835 Primary Facility 2010-05-28 Plugging(Includes Plugged/Mined Through) No Violations Noted At the time of inspection I met with Bill Shreve of Cabot. Upon inspection, the bubbling observed the previous day in between the 13 3/8¿ conductor and 9 5/8¿ casing was no longer present. A soapy water mixture was introduced around the well head and casings. There was no bubbling observed in between the casings or outside the conductor during the time of this inspection. DEP collected a video of the conditions of the well head at the time of inspection.
