1830145 Primary Facility 2009-09-21 Drilling/Alteration No Violations Noted At time of inspection Patterson Rig 251 was on site. The Site ID Sign and E/S plan were present at the entrance to the Site. During inspection I met with the Company Man, who indicated they were in the process of drilling a 12-1/4¿ hole with air to approximately 2000¿. The Company Man indicated that water was encountered at approximately 940¿, at which point they began adding soap during drilling to help keep the hole clean. Once TD is reached, 9-5/8¿ surface casing will be set and cemented to surface. I noted to the Company Man that the spray from the discharge line was traveling beyond the reserve pit and well pad are to grassy area nearby. I suggested that this spray be controlled better to direct all cuttings into the reserve pit. The Company Man said he would address this problem. A follow up inspection will be completed.
1841215 Primary Facility 2009-10-22 Follow-up Inspection Viol(s) Noted & Immediately Corrected The inspection was completed to follow up on a reported spill of drilling mud. According to the Company Man on location, approximately 10 bbls of water-based drilling mud spilled onto the well pad as a result of a mud tank overflow. The Operator had the area scraped up using hand shovels and a backhoe. The material was disposed of into the reserve pit.
1852130 Primary Facility 2009-12-17 Follow-up Inspection Violation(s) Noted : I arrived on Site on 12/16/09 at approximately 15:30. At the time of inspection, there was no drill rig on site. A backhoe operator and laborer was on site scraping up plastic lining from the well pad adjacent the reserve pit, and placing the plastic into a rolloff container for disposal. The reserve pit at the time of inspection was frozen over. Freeboard in the reserve pit was less than 2¿. A pile of drill cuttings dumped in one area of the reserve pit extended beyond the pit and onto the well pad.

2713349 Primary Facility 2018-03-20 Routine/Complete Inspection No Violations Noted Arrived onsite at 14:30. Weather overcast, ~ 38 degs. (5) wells were present on pad, the Phelps 2H, 1H, 1V, 3H and 4H. The 2H well was shut in and the other (4) wells were producing at time of insp. There was also a US Compressor unit onsite. The 1V, 1H & 2H wells were connected to compressor unit at time of insp. Visual insp. of production tanks showed moderate to heavy corrosion and rust staining at 3 out of 6 prod. tanks. (See pics on page 2). Updating and/or maintenance of prod. tanks is needed.

- Inspection of wells showed all (5) wells had the prod. tree top press. "PTTP", prod. casing press. "PCP" and prod. line press. "PLP" being monitored by wireless data sending units and/or analog gauges. - The 2H, 1H & 1V wells had (1) annular vent present labeled (9 5/8"x 5 1/2") venting to a shared poly vent tank. - The 3H & 4H wells had (2) annular vents present labeled (13 3/8"x 9 5/8") and (9 5/8"x 5 1/2") venting to a shared poly vent tank.
