The Department was on site at the A Hunsinger 2&4 Well Site to conduct a Routine Partial Inspection. At the time of the inspection, the Department noted that the E&S Inspection log indicated that Sam Wiernusz (TEC) completed a Post Rain Event Inspection. The well site is in an agriculture area. There is fencing around the perimeter of the well pad and the area outside of the fence is being used as pasture. There are two tracks from a vehicle running alongside the fence. It is presumed to be from the landowner. Also, the areas outside of the fence that do not meet temporary stabilization requirements appear to be an active agricultural area.

2496260 Primary Facility 2016-06-22 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 9:50a.m., the cellar was about half full of water with no bubbling noticed, the (9x5) vent pipe was open and venting to a tank, there was 589psi on the production casing. I took a walk around the location and everything looked OK.

I left the site at 10:35a.m.

2528827 Primary Facility 2016-10-06 Site Restoration No Violations Noted Inspection of the Hunsinger A P1 Well Pad in response to a submitted Notice of Termination request for the Erosion and Sedimentation Control Permit.

The site has been stabilized and vegetation was found to be greater than 70% uniform perennial growth. Temporary E&S BMPs have been removed from the site. No E&S problems were observed at the time of the inspection. Post Construction Stormwater Management BMPs appear to be installed properly according to the site restoration plans. The well pad is less than or equal to the size indicated on the site restoration plans.

Based on the site inspection, I recommend this notice of termination be APPROVED.
