1819259 Primary Facility 2009-08-06 Drilling/Alteration Violation(s) Noted My investigation revealed that an un-permitted discharge of diesel fuel has occurred an the site. Such a discharge is a violation of Chapter 78.54 of the Rules and Regulations of the Environmental Quality Board, 25 PA Code, § 78.54 General Requirements which provides in part: " The well operator shall control and dispose of fluids, residual waste and drill cuttings, including tophole water, brines, drilling fluids, drilling muds, stimulation fluids, well servicing fluids, oil, production fluids, and drill cuttings in a manner that prevents pollution of the waters of this Commonwealth¿"
1820595 Primary Facility 2009-08-12 Follow-up Inspection No Violations Noted At time of inspection, I met with the Company Man Gary Brown and contractor GDS. GDS was on site performing the cleanup of spilled diesel fuel which was reported and described in the previous DEP inspection dated August 8, 2009. Upon arrival on site, the drill rig and equipment had been moved off the well. GDS had already used a bulldozer to scrape some of the impacted area. I inspected the area and observed additional diesel fuel-stained areas. GDS scraped the additional soil from the pad into a pile. The contaminated soil was then loaded into a dump truck provided by Diaz Trucking for transportation and disposal. Paul Harten of GDS indicated that he will provide DEP with a copy of the disposal manifest once received. It is estimated that approximately 15cu/yds of material was scraped and removed from the site.

Operator drilling 6-1/4¿ hole and is planning to set 4-1/2¿ production casing to TD of 9970¿ (was previously proposed to be 9818¿).
1845078 Primary Facility 2009-11-10 Follow-up Inspection No Violations Noted I inspected the well head for evidence of leakage. At the time of inspection, the drill rig and equipment was still on-site and in the process of being moved off location. There was no visible bubbling observed in the cellar of this well. The horizontal portion of the well has just been completed.
1845171 Primary Facility 2009-11-04 Drilling/Alteration No Violations Noted Depth of drilling 9695'. TDproposed is 9970'. 7125' of hole is vertical and once complete will be setting packers downhole. Freeboard in pit was 6'. No visible spills. Site is lined with plastic and earthen berms is built around perimeter of pad.
1876542 Primary Facility 2010-04-13 Follow-up Inspection No Violations Noted I inspected the well head for evidence of leakage. There was standing water present in the cellar. Bubbling was observed in the cellar around the 9-5/8¿ casing. The source of the bubbling could not be determined due to the level of water in the cellar. The well is shut in and has not been placed online yet. Gas readings were collected around the wellhead and cellar using a Model 60 gas meter. No readings were detected. There was no activity on location at the time of inspection. A reserve pit is still open on site with residual cuttings and fluid. There is approximately 6 - 8' of available freeboard in the pit. The Site ID sign indicates the well as Ely 7H with permit #115-20096. According to DEP Permit files, 115-20096 is the permit number for Ely 7V. The Site ID sign should be changed to reflect the correct DEP Permit# and corresponding well name.
1890417 Primary Facility 2010-05-24 Routine/Complete Inspection No Violations Noted
2795064 Primary Facility 2018-10-26 Routine/Complete Inspection No Violations Noted Arrived on location at 08:45. Weather: Clear ~ 35 degs. with slight breeze. Onsite to initiate 72 hr. pressure build up test "PBUT" on the annuli of the Ely 1V, 5H & 7H wells. Cabot O&G has emptied well cellars to verify annuli vent connections and casing tops in cellars. The 1V & 5H wells (9 5/8"x 7") & (7"x 4 1/2") annuli and the 7H wells (9 5/8"x 4 1/2") annulus have been isolated and have been venting to onsite poly vent tanks. All wells conductors had open top casings in cellar.

(See individual well insp. pictures for cellar details). - Upon arrival, I met with Cabot O&G Rep. Sully Rutledge. Prior to shutting in wells annuli, initial gas screenings and flow checks were performed at the wells annuli using the Depts. Altair 5X gas meter, calibrated 10/18/18, and Cabots' RS-484 Alicat "Whisper" flow meter. Pre-shut in screenings showed gas and flow detected at the following annui: - 1V: (7"x 4 1/2") = 100% CH4, 30.5 cf/d

       (9 5/8"x 7") = 14% CH4, 2.0 cf/d *

- 5H: (7"x 4 1/2") = 8% CH4, 2.0 cf/d *

2796584 Primary Facility 2018-10-29 Routine/Complete Inspection Pending Arrived on location at: 08:45. Weather: Overcast ~ 38 degs. Onsite to conclude the 72-hr. pressure build up test ¿PBUT¿ being performed on the Ely 1V, 5H & 7H wells annuli.

- Upon arrival I met with Cabot O&G Reps. Jeremy Rapp and Sully Rutledge. Selman and Associates Reps. Kaydee Oliver and Tyler Thomas and accompanied by DEP Geologist, Eric Rooney. There was also a GDS Vac Truck and Operator onsite to add or remove water to cellar areas as needed. The ending PBUT pressures were checked by Cabot Reps. using a digital Crystal pressure gauge. Ending pressures are listed below: - 1V: (7"x 4 1/2") = 6.0 psi.*, Gas samples collected = 100% CH4.; (9 5/8"x 7") = 23.5 psi.*, Gas samples collected = 100% CH4. Cellar was full of water, no bubbling was noted. - 5H: (7"x 4 1/2") = 18.0 psi.*, Gas samples collected = 100% CH4; (9 5/8"x 7") = 3.5 psi., Gas samples collected = 100% CH4. Cellar was full of water. No bubbling was noted. - 7H: (9 5/8"x 4 1/2") = -5.0., Gas samples collected = 100% CH4; This is a (2) string w

2805453 Primary Facility 2018-11-05 Follow-up Inspection Pending Arrived onsite at 13:45. Onsite to perform follow-up inspection of the Ely 7H well cellar area. Upon arrival I met with Cabot O&G Rep., Steve Watson, Selman & Assoc. Reps., Tyler Thomas and Kaydee Oliver and DEP Rep., Jonathan Sassi.

Upon inspection of the 7H well cellar, all previous mud residue had been removed from cellar bottom. Lower wellhead components, cellar wall and floor have been pressure washed. Clean, intact cement cellar floor was visible through approx. 1" of clear water at bottom of cellar. All removed cellar material has been stored in containment onsite awaiting classification for proper disposal. It appears that all possible contaminants had been contained to the Ely 7H well cellar only. No visual signs of possible contaminants were noted outside Ely 7H well cellar area at time of reinspection. All present took lap around location and no further issues were noted indicating additional sample collection was warranted. Left location at 15:00.
