1811419 Primary Facility 2009-07-08 Drilling/Alteration No Violations Noted Drill rig on site is SWJ-15. Depth of drilling at time of inspection was TD 9332'. At the time of inspection, the crew was tripping out drill pipe. Crew will be settin 4-1/2 casing at TD. The Company Man indicated they will cement at the top in the straight hole, and use packers downhole. Drillers log was inspected and was filled out properly.
1819251 Primary Facility 2009-07-31 Drilling/Alteration No Violations Noted At time of inspection I met with Company Man, Larry Fulmer. According to Mr. Fulmer, they were in the process of running tubing down wells 1H and 2H for fracking. Key Industries was set up on Ratzel 1H running coil tubing, and a drill rig was running straight tubing on Ratzel 2H. Mr. Fulmer indicated that the plan was to perforate all three wells on location (1H, 2H and 3V) and then frac the wells the following week.
1820619 Primary Facility 2009-08-13 Drilling/Alteration No Violations Noted At time of inspection I met with Company Man, Stephen Knight. According to Mr. Knight, they were in the process of flowing back frac water from Ratzel 1H. Flowback or frac water was reported to be approximately 30bbls/hr. A service rig was set up on Ratzel 2H. Mr. Knight stated that a squeeze cement job was being performed on a section of casing where a leak was suspected. Once the cement job is complete, the casing will be retested prior to fracking. While on site, Haliburton was mobilizing fracking equipment to the location. Mr. Knight indicated that fracking of the wells will resume during the week of August 17th.
1845083 Primary Facility 2009-11-10 Follow-up Inspection No Violations Noted I inspected the well head for evidence of leakage. There was no standing water present in the cellar and no visible bubbling observed. The well is currently on-line.
1846444 Primary Facility 2009-11-24 Routine/Complete Inspection No Violations Noted I inspected the well head for evidence of leakage. At the time of inspection there was visible bubbling in the cellar. It appears there is a leak around one of the fittings on the 7" vent pipe. There is also evidence of bubbling coming from another source below the vent pipe, however, due to the level of water present in the cellar, the source of the other leak could not be seen. The well is currently on-line.

Gas readings were collected around the well head using a Model 60 gas meter. No readings were detected. There was no activity on site. The site has not been restored.

Gurgling heard in one of the pipes connected to the well head. DEP will follow up with operator to determine source of gurgling. No one on site at time of inspection.

2528782 Primary Facility 2016-10-12 Site Restoration No Violations Noted Inspection of the Ratzel P1 Well Pad in response to a submitted Notice of Termination request for the Erosion and Sedimentation Control Permit.

The site has been stabilized and vegetation was found to be greater than 70% uniform perennial growth. Temporary E&S BMPs have been removed from the site. No E&S problems were observed at the time of the inspection. Post Construction Stormwater Management BMPs appear to be installed properly according to the site restoration plans. The well pad is less than or equal to the size indicated on the site restoration plans.

Based on the site inspection, I recommend this notice of termination be APPROVED.

2791952 Primary Facility 2018-10-19 Routine/Complete Inspection No Violations Noted Arrived on location at 09:00. Weather: Clear ~ 35 degs. with slight breeze. Onsite to initiate 72 hr. pressure build up test "PBUT" on the annuli of the Ratzel 1H, 2H & 3V wells. Vac Truck had emptied well cellars to verify annuli vent connections. All wells (9 5/8"x 7") & (7"x 4 1/2") annuli have been isolated and were venting to onsite poly vent tanks. All wells (20"x 13 3/8") & (13 3/8"x 9 5/8") cemented annuli had open top casings in cellar. The 2H & 3V wells open casings tops were beneath the cement floor of cellars. (See individual well insp. pictures for cellar details).

- Upon arrival, I met with Cabot O&G Rep. Jeremy Rapp. Prior to shutting in wells annuli, initial gas screenings and flow checks were performed at the wells annuli using the Depts. Altair 5X gas meter, calibrated yesterday, and Cabots' RS-484 Alicat "Whisper" flow meter. Pre-shut in screenings showed gas and flow detected at the following annui: - 2H: (7"x 4 1/2") = 100% CH4, 575 cf/d - 3V: (7"x 4 1/2") = 100% CH4, 18.5 cf/d

2795878 Primary Facility 2018-10-22 Routine/Complete Inspection No Violations Noted Arrived on location at: 08:45. Weather: Overcast ~ 38 degs. Onsite to conclude the 72-hr. pressure build up test ¿PBUT¿ being performed on the Ratzel 1H, 2H & 3V wells annuli.

- Upon arrival I met with Cabot O&G Reps. Jeremy Rapp and Selman and Associates Reps. Kaydee Oliver and Tyler Thomas. There was also a GDS Vac Truck and Operator onsite to add or remove water to cellar areas as needed. The ending PBUT pressures were checked by Cabot Rep. using a digital Crystal pressure gauge.Ending pressures are listed below: - 1H: (7"x 4 1/2") = 0.0 psi., No gas detected.; (9 5/8"x 7") = -2.2 psi., No gas detected. Cellar was full of water, no bubbling was noted. - 2H: (7"x 4 1/2") = 170.1 psi., Gas sample taken = 100% CH4; (9 5/8"x 7") = -3.1 psi., No gas detected. Cellar was full of water. No bubbling was noted. - 3V: (7"x 4 1/2") = 61.0 psi.*, Gas sample taken = 100% CH4; (9 5/8"x 7") = 24.2 psi.*, Gas sample taken = 100% CH4. Cellar was full of water, no bubbling noted.
