The Department was on site at the A Hunsinger 1&3 Well Site to conduct a Routine Partial Inspection. At the time of the inspection, the E&S Inspection Log indicated that a Post Rain Event Inspection was completed on 5/13/16 by Sam Wiernusz (TEC). During the course of the inspection, it was noted that there are two areas that do not meet temporary stabilization requirements. One of the areas appears to have been used by the landowner. Operator, please review these areas and make any necessary changes. Also, a tracked road was noted to have been originating from the site. This appears to have been created by the landowner to access their property.

2496254 Primary Facility 2016-06-22 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 9:50a.m., the cellar was full of water with no bubbling noticed, the (9x5) vent pipe was open and venting to a tank, there was 528psi on the production casing. I took a walk around the location and everything looked OK.

I left the site at 10:35a.m.
