2109462 Primary Facility 2012-10-19 Incident- Response to Accident or Event Pending Arrived at the site at 12:35 in response to a spill of (15) gallons of production fluid that occurred on 10-17-12. Brody Webster of Cabot Oil and Gas reported the spill to the NERO ERT at 20:43 on 10-17-12. The report indicated the (15) gallons of brine spilled within an earthen containment area around tanks. During the inspection I observed a dura skim plastic containment liner covering the excavation of the impacted area where the spill occurred, inside the earthen containment berm for the production fluid tanks at the site. A Diaz owned lined roll-off container located just outside the earthen containment berm where the spill occurred, contained the excavated material along with some plant trash. I used a Extech, ExStik (2) pH, Conductivity, Salinty and TDS meter to field screen several areas of puddled water in a relatively close proximity where the spill occurred. All conductivity readings indicated less than (500) microSiemens/cm.

2530992 Primary Facility 2016-10-20 Site Restoration No Violations Noted Inspection of the Grimsley J P1 in response to a submitted Notice of Termination request for the Erosion and Sedimentation Control Permit. EPS, Jon Sassi, was also present on this inspection.

The site has been stabilized and vegetation was found to be greater than 70% uniform perennial growth. Temporary E&S BMPs have been removed from the site. No E&S problems were observed at the time of the inspection. Post Construction Stormwater Management BMPs appear to be installed properly according to the site restoration plans. The well pad is less than or equal to the size indicated on the site restoration plans.

Based on the site inspection, I recommend this notice of termination be APPROVED.

Jon and I also conducted a general inspection of the pad. Signs and labeling appeared to be correct. The mailbox contained the required items. There were no apparent issues with containment. Conductivity readings of water on the pad were of no concern.

2788904 Primary Facility 2018-10-01 Routine/Complete Inspection Pending Arrived on location at 09:00. Weather: ~ 65 degs., Overcast with slight breeze. Onsite to initiate 72 hr. pressure build up test "PBUT" on the annuli of the Grimsley 1V & 2H wells. GDS Vac Truck was onsite to empty well cellars to verify annuli vent connections. Both wells, (9 5/8"x 5 1/2") annuli have been isolated and were venting to onsite poly vent tanks. Both wells (16"x 9 5/8") cemented annuli had open top 16" casings in cellar. No bubbling was noted in well cellars prior to pump out.

- Upon arrival, I was accompanied by DEP Geologist, Eric Rooney and Cabot O&G Reps. Ben Whitaker and Keenan Small. Prior to shutting in wells annuli, initial gas screenings and flow checks were performed at the 1V & 2H annuli using the Depts. Altair 5X gas meter, calibrated this morning, and Cabots' RS-484 Alicat "Whisper" flow meter. Pre-shut in screenings showed: - 1V: (9 5/8"x 5 1/2") = 100% CH4, 100.3 cf/d - 2H: (9 5/8"x 5 1/2") = 100% CH4, 187.0 cf/d

- Both wells (9 5/8"x 5 1/2") annuli were shut in to start 72 hr P

2788906 Primary Facility 2018-10-04 Routine/Complete Inspection Pending Arrived on location at 08:45. Onsite to conclude 72 hr PBUT being performed on the Grimsley 1V & 2H wells (9 5/8"x 5 1/2") annuli. I was accompanied by D.E.P. Reps. Eric Rooney, Geologist and Ulyana Trenovich, OGI. Cabot O&G Reps. Ben Whitaker and Keenan Small and Selman and Associate Reps. Kadee Oliver and Tyler Thomas.

Ending pressure readings were checked at ~ 09:00 and are listed below: - Grimsley 1V: (9 5/8"x 5 1/2") = 49.0 psi. Samples collected at ~ 09:10. - Grimsley2H: (9 5/8"x 5 1/2") = 269.4.0 psi. Samples collected at ~ 09:20. Water levels in both well cellars was full at time of inspection. Bubbling was noted in the Grimsley 1V cellar. A GDS vacuum truck was on location to pump out well cellars. The 1V cellar water level was pumped down revealing origin of bubbling at top seam of 9 5/8" casing fitting. (See pic #1 for 1V) Water level was pumped down further showing no further bubbling noted beneath this casing seam. (See pic #2 for 1V). A gas sample was collected over the bubbling area at ~ 09:1
