1822389 Primary Facility 2001-08-20 Follow-up Inspection Viol(s) Noted & Immediately Corrected On August 20, 2009 at approximately 10:00 am, I received a call from Paul Harten of GDS reporting a spill of approximately 25 gallons of drilling mud on location. The spill occurred due to a leaking mud hose on the rig. It was reported that the drilling mud was immediately contained and cleaned up and the leaking hose was replaced. Mr. Harten indicated that the area where the drilling mud spilled was scraped with a backhoe, and the soil and mud was disposed of in the reserve pit on site. At the time of inspection, the area appeared to be clean and free of drilling mud. Some of the spilled drilling was contained in the cellar of the well. This mud was being pumped out and into the reserve pit. My investigation revealed that an un-permitted discharge of drilling mud has occurred at the site. Such a discharge is a violation of Chapter 78.54. Violation was Immediately Corrected.

1845077 Primary Facility 2009-11-10 Follow-up Inspection No Violations Noted I inspected the well head for evidence of leakage. There was no standing water present in the cellar. No bubbling was observed. The well has not yet been connected to pipeline.
1876660 Primary Facility 2010-04-13 Follow-up Inspection No Violations Noted I inspected the well head for evidence of leakage. The cellar was full of water at the time of inspection. No bubbling was observed. A 2" vent riser which extends approximately 6' above the cellar is present. Gas readings were collected from the vent pipe using a Model 60 Methane gas detector. A reading of 3.5% methane was detected from the vent pipe. Due to the amount of water present in the cellar, it could not be determined what the pipe was connected to. The well is shut in and has not yet been connected to pipeline.

2123797 Primary Facility 2012-12-12 Complaint Inspection No Violations Noted Arrived at the site at 11:45 in response to an alleged complaint of leaking brine tanks. The Department received the complaint on 12-11-12 at 10:35. I was accompanied by Briana Cunningham, DEP WQS. During the inspection the Department did not observe any evidence of leaking brine tanks or brine spills at the site. I used an an Extech, ExStik (2) pH, Conductivity, Salinty and TDS meter to field screen areas of ponded water inside of the earthen containment berm where the production fluid tanks are stored at the site. I recorded a conductivity reading of (163) microSiemens/cm inside the earthen containment berm, (404) microSiemens/cm just outside the berm close to the wells on-site and (306) microSiemens/cm in an area of ponded water behind the tanks. I also observed aquatic life in this area of ponded water, little to no turbidity in all areas of ponded water and had to break ice to record readings. Departed from the site at 13:00.
2795060 Primary Facility 2018-10-26 Routine/Complete Inspection Pending Arrived on location at 08:45. Weather: Clear ~ 35 degs. with slight breeze. Onsite to initiate 72 hr. pressure build up test "PBUT" on the annuli of the Ely 1V, 5H & 7H wells. Cabot O&G has emptied well cellars to verify annuli vent connections and casing tops in cellars. The 1V & 5H wells (9 5/8"x 7") & (7"x 4 1/2") annuli and the 7H wells (9 5/8"x 4 1/2") annulus have been isolated and have been venting to onsite poly vent tanks. All wells conductors had open top casings in cellar.

(See individual well insp. pictures for cellar details). - Upon arrival, I met with Cabot O&G Rep. Sully Rutledge. Prior to shutting in wells annuli, initial gas screenings and flow checks were performed at the wells annuli using the Depts. Altair 5X gas meter, calibrated 10/18/18, and Cabots' RS-484 Alicat "Whisper" flow meter. Pre-shut in screenings showed gas and flow detected at the following annui: - 1V: (7"x 4 1/2") = 100% CH4, 30.5 cf/d

       (9 5/8"x 7") = 14% CH4, 2.0 cf/d *

- 5H: (7"x 4 1/2") = 8% CH4, 2.0 cf/d *

2796580 Primary Facility 2018-10-29 Routine/Complete Inspection Pending Arrived on location at: 08:45. Weather: Overcast ~ 38 degs. Onsite to conclude the 72-hr. pressure build up test ¿PBUT¿ being performed on the Ely 1V, 5H & 7H wells annuli.

- Upon arrival I met with Cabot O&G Reps. Jeremy Rapp and Sully Rutledge. Selman and Associates Reps. Kaydee Oliver and Tyler Thomas and accompanied by DEP Geologist, Eric Rooney. There was also a GDS Vac Truck and Operator onsite to add or remove water to cellar areas as needed. The ending PBUT pressures were checked by Cabot Reps. using a digital Crystal pressure gauge. Ending pressures are listed below: - 1V: (7"x 4 1/2") = 6.0 psi.*, Gas samples collected = 100% CH4.; (9 5/8"x 7") = 23.5 psi.*, Gas samples collected = 100% CH4. Cellar was full of water, no bubbling was noted. - 5H: (7"x 4 1/2") = 18.0 psi.*, Gas samples collected = 100% CH4; (9 5/8"x 7") = 3.5 psi., Gas samples collected = 100% CH4.
