The Site ID and E/S Plan were present at the entrance of the access road. At the time of the inspection, the well was shut down. The drillers had reported a large kick of gas at 6950', at which time a small amount of drilling mud was sprayed on some nearby trees just off the drill pad. The B.O.P. was used to shut the well down as a result of the gas kick. The well was enventually controlled using mud, and the spill of drilling mud was cleaned up and contained.

Tetra well services was on Site cotrolling flowback by flaring the well. According to the company man, they had just recently set the 7" casing, and ran cement down the hole. When they bumped the plug during the cement job, they started getting gas immediately to the surface. The company man indicated that it was unclear where the gas was coming from (deep/shallow), so a gas sample had been collected by the operator and was being sent out for analysis. The company man said that Tetra will continue to flare the well until futher notice.

2175563 Primary Facility 2013-06-03 Incident- Response to Accident or Event No Violations Noted Friday 5/31/13, the operator had excavated an area that was approximately 30’X30’X5.5” deep; totaling 15.25 yards of potentially impacted material removed. The same day, the operator had a consultant collect confirmatory samples and background samples that should be completed in one week’s time. The estimated volume of fluid released from the Gesford 5H was determined to be 12 bbl. During the inspection, five (5) roll off containers were situated on site. Four (4) of which contained approximately fifteen (15) tons of recovered fill material each. The 500bbl tank was in place of the production tanks which had been removed to facilitate the excavation of the contaminated fill material. At the time of the release, the production tanks were located in an earthen berm on site. Currently the 500bbl tank is situated on makeshift plastic containment. The excavation was covered in plastic at the time of the inspection. NERO EC&B deficiency memo dated 9/2/14 sent to company 9/22/14.
2484654 Primary Facility 2016-05-11 Routine/Partial Inspection No Violations Noted The Department, Jon Sassi (DEP-EPS) and I were on site to conduct a Routine Partial Inspection. At the time of the inspection, it was noted that the well site appears to meet temporary stabilization requirements. The Department noted that one of the production tanks has significantly more rust colored staining on it than the other. Please investigate the origin of the staining on the production equipment.
2509184 Primary Facility 2016-08-04 Complaint Inspection No Violations Noted At time of inspection, arrived on site at 11:55a.m. pertaining to a complaint called into the department about a backhoe and a dumpster being hauled onto the site and unknown activity. There was no activity on this site at time of inspection. There was no backhoe on site but there were two role off containers on the Castello J Pad 1 site. The marks on the entrance road indicated that the containers have recently been dropped off.

I left the site at 12:05p.m.

2625959 Primary Facility 2017-08-16 Routine/Complete Inspection No Violations Noted The Gesford access road was in good shape, but the Cabot locks on the gate did not fit the key the Department has. The pad surface is in good shape and solid. The cut and fill slopes were stable and well vegetated with perennial vegetation. There was no bubbling in the well cellars and there were no erosional issues noted.

There were no violations observed.

2642773 Primary Facility 2017-10-02 Site Restoration No Violations Noted I inspected the Gesford K, P1 well site on October 2, 2017. It is situated at the end of a long access road. The road was solid and stable as was the pad surface. The cut and fill slopes were well vegetated, although just outside the LOD the landowner was installing fence and regrading things a bit. These activities were not affecting the pad stability. There did not appear to be any erosional issues at this time.

I recommend APPROVAL of this NOT.
