No standing water was present in the cellar and no visible bubbling observed. The cellar has been removed and the area around the base of the well has been filled to grade with P-gravel. The vents on the 9-5/8" and 7" casings were open. The well is online. Based on visual observations, no leakage was detected. Gas readings were collected around the well head using a Model 60 gas meter. No readings were detected. There was no activity on site. The site has not been restored.

The Department conducted a Routine Partial Inspection at the Gesford Pad 2 Well Site. The E&S Inspection Log indicated that Mike Brilla (TEC) completed a Post Rain Event Inspection on 12/15/15. At the time of the inspection, the site appeared to meet temporary stabilization requirements. It was noted that there was black material alongside the lines running to the production tanks. Also, there was what appeared to be recently disturbed pad surface adjacent to the production containment.

2548417 Primary Facility 2016-12-01 Routine/Complete Inspection No Violations Noted The Department was on site to conduct a Routine Complete inspection for the Gesford P2 well site (Erosion and sediment permit 58-09-802). The wells associated with this site are as follows:

The site was well vegetated with perennial grass, and there were no erosion or sedimentation issues with the site. This site is stable at this time, and there were no issues to report.

2771572 Primary Facility 2018-09-04 Routine/Complete Inspection Pending Arrived on location at 09:00. Onsite to initiate a 72 hr. pressure build up test "PBUT" on the annuli of the Gesford 2V and 7H wells. Both wells annuli have been isolated and were venting to onsite poly vent tanks.

- Upon arrival I met with Cabot O&G Rep. Jeremy Rapp. Prior to shutting in wells annuli, initial gas screenings and flow checks were performed using the Depts. Altair 5X gas meter, calibrated this morning, and Cabots' RS-484 Alicat "Whisper" flow meter. Pre-shut in screenings showed gas and flow detected at both wells cemented annuli. - 2V: (9 5/8"x 7") = 100% CH4; 69.8 cf/d * - 7H: (13 3/8"x 9 5/8") = 100% CH4; 435 cf/d * Both wells annuli were shut in to start 72 hr PBUT at approx. 09:15. We will meet back on location to conclude PBUT on Friday 9/7/18 and collect gas samples off annuli, where present, for isotopic analysis. Left location at 09:40. (Please see attached well bore diagrams for the Gesford 2V and 7H wells for further details.)

2772765 Primary Facility 2018-09-07 Routine/Complete Inspection Pending Arrived on location at 09:00. Onsite to conclude the 72 hr. pressure build up test "PBUT" being performed on the annuli of the Gesford 2V and 7H wells. Upon arrival I met with Cabot O&G Rep. Jeremy Rapp. and Selman and Associates Rep. Kaydee Oliver. At approx. 09:15 the ending PBUT pressures were checked by Mr. Rapp using a Crystal digital pressure gauge. Ending pressures are listed below:

- 2V: (7"x 4 1/2") = -5.9 psi.; (9 5/8"x 7") = 94.7 psi.* - 7H: (9 5/8"x 4 1/2") = 0.0 psi.; (13 3/8"x 9 5/8") = 162.7 psi.*

(*) The presence of venting gas and pressure detected in the Gesford 2H and 7H cemented annuli appears to be indicative of Defective Cement and/or Defective Casing. Since this condition was simultaneously detected by the Dept. and Cabot O&G, no violation for 78a.86 will be issued at this time. Please respond to the Dept. within 30 days of receipt of this inspection with a plan how Cabot O&G plans to address this defective condition per 78a.86.

While checking ending PBUT pressures, continuous bubbli

2785741 Primary Facility 2018-09-24 Follow-up Inspection Pending Arrived on location at 09:00. Onsite to conclude this repeat / follow-up 72 hr. pressure build up test "PBUT" being performed on the annuli of the Gesford 2V and 7H wells. Since conclusion of the last PBUT on these wells, concluded on 9/7/18, Cabot has emptied well cellars, verified and tightened all annular vent line connections. On 9/21/18 Dept. Rep., Eric Rooney met with Cabot Reps. to perform initial gas checks and annular vent flows prior to starting PBUT. (See Erics notes of this insp. attached, Photo #2). Summary of detected results below:

- Gesford 7H: 4 ½¿ x 9 5/8¿ annulus = 100% LEL, 100% methane, flow = 422 cf/d, annulus was shut in at 0932 hours - Gesford 2V: 7¿ x 9 5/8¿ annulus = 100 LEL, 100% methane, flow = 71 cf/d, annulus was shut in at 0945 hours

                2V: 4 ½¿ x 7¿ annulus = 100% LEL, 3% methane peak dissipated to 0, flow = fluctuating between 0 cf/d ¿ 1.5 cf/d, annulus was shut in at 0950 hours.

- Upon arrival today, I met with Cabot O&G Rep. Jeremy Rapp. and Selman and Associ

2835007 Primary Facility 2019-02-04 Drilling/Alteration Pending Arrived on location at 08:30. Onsite to check on progress of remedial activities underway on the Gesford 2H well. Weather: Clear, mild ~ 40 degs. Upon arrival I met with WSS, James Restley. Workover "WO" Rig had moved onto location on Jan. 18th. Prod. tubing was pulled. GAPS and CBL logs were ran and interpreted. WO activities resumed on Jan. 30th. To date, 4 1/2" casing was cut and pulled at 1050'. 7".casing was perf'd at ~ 1030', no injection rate could be established.
 - Present plan moving forward is to mill top of 4 1/2" casing stub down to depth of 7" casing shoe at 1575'. Additional CBL will be ran to  gather further data to develop plan for next remedial efforts.
 - Cabot will continue to keep The Department informed as remedial work progresses.

The Gesford 7H well will remain shut in during remedial efforts. An additional PBUT to be ran on the 7H wells (13x9) annulus when conductor cap installation is completed.

2840310 Primary Facility 2019-02-11 Drilling/Alteration Pending Arrived on location at 08:30. Onsite to check on progress of remedial activities underway on the Gesford 2V. Weather: Clear, cold ~ 20 degs. Upon arrival I met with WSS, James Restley. Workover "WO" Rig had moved onto location on Jan. 18th. Prod. tubing was pulled. GAPS and CBL logs were ran and interpreted. WO activities resumed on Jan. 30th. 4 1/2" casing was cut and pulled at 1050'. 7" casing was perf'd at ~ 1030', no injection rate could be established. Cabot's plan moving forward is to mill ~ 600' of 4 1/2" casing to below the 9 5/8" casing shoe.
 - Presently, rig has milled ~ 22' of casing and is changing out mill. This  will be third mill used. Progress has been slow but good metal and cement returns have been seen.
 - Cabot will continue to keep The Department informed as remedial work progresses.
 - The Gesford 7H well will remain shut in during remedial efforts. Mr. Restley confirmed that a conductor cap had been welded atop the 13 3/8"x 9 5/8" annulus. A newly installed 1" pipe was visible a
2846544 Primary Facility 2019-02-20 Drilling/Alteration Pending Arrived on location at 09:00. Onsite to check on progress of remedial activities underway on the Gesford 2V. Weather: Clear, cold ~ 20 degs. Upon arrival I met with WSS, James Restley. Workover "WO" Rig had moved onto location on Jan. 18th. Prod. tubing was pulled. GAPS and CBL logs were ran and interpreted. WO activities resumed on Jan. 30th. 4 1/2" casing was cut and pulled at 1050'. 7" casing was perf'd at ~ 1030', no injection rate could be established. Cabot's plan moving forward is to mill out 4 1/2" casing and perform additional perf and squeeze at a lower depth, to be determined.
 - Presently, rig has milled ~ 100' of 4 1/2" to ~ 1148'. Rig has perf'd 7" casing at 1030', 1027' and most recently at ~ 1115'. No injection rate could be established. Last PT performed on 7" held 1500 psi for 14 hrs showing a gradual press drop to ~ 750 psi. Cabot has decided to spot cement plug accross perfs with calculated TOCP at ~ 800'. 1500 psi will be held on 7" overnight.
