1847933 Primary Facility 2009-12-04 Follow-up Inspection Violation(s) Noted At the time of inspection, construction crews were on location fixing the liner in the new reserve pit, and re-working the well pad in preparation for the next proposed well. While on site, I inspected the existing reserve pit. The freeboard in the reserve pit was less than 6". This is a violation of 25 PA Code 78.56(a)(2) which provides: " A pit shall be designed, constructed and maintained so that at least 2 feet of freeboard remain at all times."
1852357 Primary Facility 2009-12-10 Follow-up Inspection No Violations Noted I arrived on site at approximately 8:00 am to perform a follow-up inspection of the reserve pit associated with the Baker 1 Well. A previous DEP inspection of this location dated 12/4/09 resulted in issuing a violation for failure to maintain adequate freeboard in the pit. At the time of this inspection, the reserve pit had been encapsulated and the surface restored to the grade of the existing well pad. A letter received from Cabot Oil and Gas dated 12/22/09 indicates that on 12/03/09 cement was added to the pit and any excess fluid was removed from the pit prior to solidification. The letter further indicates the encapsulation of the pit was completed on 12/05/09. Based on this information, the violation cited on 12/04/09 has been corrected.

1876534 Primary Facility 2010-04-13 Follow-up Inspection No Violations Noted At the time of inspection the well was shut in and off-line due to drilling activites associated with Baker #3. There was no standing water present around the wellhead. I poured water around the base of the wellhead and observed bubbling around the outside of the 9-5/8" and 7" casings. Gas readings were collected using a Model 60 gas meter. No readings were detected. Crews were on location breaking down Patterson Rig 154 and loading onto transport trucks. The reserve pit was still open and had sufficient freeboard.

A 4' high piece of 4" casing has been installed as a monument with the DEP Permit # welded on a plate attached to the riser. The ground around the well has been leveled off to grade.
1887839 Primary Facility 2010-05-27 Plugging(Includes Plugged/Mined Through) No Violations Noted At the time of inspection there was no activity on site. The well has been plugged. The conductor and surface casings were exposed at the ground surface. There was no standing water present around the well head. A soapy water mixture was introduced around the well head and casings. During the time of inspection, there was evidence of bubbling observed in between the 9 5/8" and 13 3/8" conductor. The bubbling was localized to an area located on the western side of the well head. Along the outside of the conductor, some very small bubbling was also observed. The bubbling in this area was consistent with 1 bubble coming to surface approximately every second. DEP collected a video of the conditions of the well head at the time of inspection.

1898256 Primary Facility 2010-06-22 Plugging(Includes Plugged/Mined Through) No Violations Noted At the time of inspection I met with Johnny Crawford who is overseeing plugging of well. Stone Service Rig is rigged down. Plugging of well is complete. According to Mr. Crawford, top of cement of last plug was taggedat 695' in the 7" casing. The remaining portion of the 7" casing is filled with p-gravel. A plate with a vent has been welded on top of the 9 5/8" casing to cover the space between the 9 5/8" casing and 7" casing. A 4 ' section of 4" casing has been welded on top of the plate which has a metal plate containing the DEP permit number on it 115-20026. Crews on site continue to clean up site and de-mobilize equipment off location.

2641301 Primary Facility 2017-09-28 Site Restoration No Violations Noted I inspected the Baker pad on September 28, 2017 for Notice of Termination (NOT). The site's access road was smooth and solid, with a cornfield bordering the one side. The site itself was well vegetated and nicely maintained, mowed neatly and compact in size. The vegetation was all healthy and exceeded the 70% perennial vegetative threshold. The pad surface was smooth and not rutted and there were no erosional issues present at the time of inspection.

I recommend APPROVAL of this NOT.
