The Site ID sign and E/S Plan were present at the entrance of the access road. At the time of inspection, there was no drilling activity on site.

The inspection was completed as a follow up of a previous inspection performed on 9/25/09 where gas seepage was observed in the cellar of the well. During this inspection, there was only a couple of inches of water on the floor of the cellar. Bubbling was observed coming to the surface around the 16" conductor.

Visible bubbling was observed along the outside of the conductor. Due to the level of water in the cellar, it could not be determined where the leakage was coming from. The well is currently on-line.

1876536 Primary Facility 2010-04-13 Follow-up Inspection No Violations Noted I inspected the well head for evidence of leakage. There was standing water present in the cellar. Visible bubbling was observed coming out of the end of the 9-5/8" vent which was submersed. Bubbling was also observed coming up to surface along the outside of the 9-5/8" casing. Due to the water level in the cellar, it could not be determined where the bubbling was originating from (between 9-5/8" and conductor or outside conductor). The well was shut in and taken offline. A new well head has been placed on the well. Gas readings were collected using a Model 60 gas meter. No readings were detected.

Bubbling still present in both cellars (Gesford 3 and 9) and time of inspection.

I requested information on compressive strength of the blend of cement being used.

Squeezed in 2 bbls into perforations behind 4 1/2" casing with 1600 psi. i bbl was left above cement retainer. Crew tried to pull 4 1/2" casing and was not successful. At time of inspection, crew was on standby. During inspection gurgling was heard in cellar of well.
1883228 Primary Facility 2010-05-06 Plugging(Includes Plugged/Mined Through) No Violations Noted Running injection rate test to determine injection rate on 4" casing. Perforations have been made in the 4" with 8H/1320-1322' (kb) with TAG Gun. A cement retainer was set at 1295'. Crews TIH with tubing and tried to pump in 3 bbls of water with 15% acetic acid. While on site, injections tests were completed. Basd on the initial injection tests, the formation was not taking any cement. Additional attempts to squeeze cement will be made.
1883276 Primary Facility 2010-05-06 Drilling/Alteration No Violations Noted I Met with Mark Howard of Boots n Coots and Art Stewart from Cabot on the site. GDS Rig 15 is rigged up over well and Superior Well Services was providing cement and pumping services. Mr. Howard stated that the 7" and 4 1/2" casings had been cut at 900'. Two series of injection tests had been performed using freshwater and 15% acetic acid. As I arived on site, they were in the middle of another attempt to squeeze in cement. The crew tripped in hole with tubing and pumped in 3.2 bbls of cement in tubing. With 800lbs of pressure, they were able to pump in 1.3 bbls of cement through the cut section of pipe. Approximately 90' of cement was left above retainer. Mr. Howard mentioned that a pressure gauge and a Barton recorder set up on the 7" x 4 1/2" annulus have showed 0 psi consistently for the past two days with no change. A gauge was also installed on the bull plug on the 9 5/8" x 13 3/8" annulus. Crew will be waiting on cement 24 hrs before resuming plugging activities.
1886412 Primary Facility 2010-05-24 Plugging(Includes Plugged/Mined Through) No Violations Noted At the time of inspection I met with Mark Howard of Boots and Coots who has been overseeing plugging activities for Cabot. Mr. Howard summarized the recent activities. The 7" and 9 5/8" casings were mechanically cut at 300'. An injection rate could not be established at this depth, and therefore no cement was squeezed into the formation. Cement was spotted at 300', and top of cement was tagged at 238'. The top of the 7" casing was cut approximately 1-2" below the top of the 9 5/8" casing. The remaining open sections of casings were then filled to surface with gravel. A metal plate was welded on top of the 9 5/8" casing with a 2" port. A 2" riser with a bull plug and needle valve was screwed into the metal plate. A section of 4 1/2" casing was welded to the well extending approximately 4' above surface. A metal plate containing the well permit number was welded to the top of the 4 1/2" riser. Mr. Howard reported the plugging activities on this well were completed at approximately 18:30 pm on 5/23/10.
1886581 Primary Facility 2010-05-18 Plugging(Includes Plugged/Mined Through) No Violations Noted Mark Howard, Art Stewart on site. GDS Rig 15. A balanced plug was set at 350' to 425'. Pressured up the 7" with 300 lbs and had 0 psi on the 9 5/8 x 7" annulus. Pressrue bled down two times to 90 psi on 7". pressured up to 300 lbs a third time and let sit overnight. Gauge reading in the morning was 90 psi. Have established no injection rate into cut. Today plan to mechanically cut 7" and 9 5/8" casing at 300' and try to squeeze cement in at that depth.
1887831 Primary Facility 2010-05-27 Plugging(Includes Plugged/Mined Through) No Violations Noted At the time of inspection, there was no activity on site. All drilling equipment has been mobilized off location. The cellar of the well had approximately 1¿ of standing water. Intermittent bubbling was observed on the outside of the 9-5/8¿ casing. This bubbling occurred approximately every 30 seconds to 1 minute and consisted of several bubbles at each time. This bubbling was observed to be southwest of the wellhead. Minor bubbling was also observed on the northern side of the well head and appeared to be coming from the welded plate on the 9 5/8¿ casing. The exact source of the bubbling in this area, however, could not be clearly determined given the depth of the cellar and cloudiness of the water. A 100 lb pressure gauge installed on the 2¿ riser connected to the 9 5/8¿ casing showed 0 psi at the time of inspection. DEP collected a video of the conditions of the wellhead during this inspection.
1887832 Primary Facility 2010-05-28 Plugging(Includes Plugged/Mined Through) No Violations Noted At the time of inspection I met with Bill Shreve of Cabot. The standing water had been removed from the cellar, exposing the 20¿ and 13 3/8¿ casings. Upon inspection, it was determined that the intermittent bubbling observed during previous inspections, was coming from in between the 20¿ conductor and 13 3/8¿ casing. The bubbling continued to be intermitted with several bubbles coming to surface about once every 30 seconds to 1 minute. The bubbling observed the previous day on the northern side of the well head was no longer present. A soapy water mixture was introduced around the well head and casings. During the time of inspection, there was no evidence of bubbling on the northern side of the casing. DEP collected a video of the conditions of the well head at the time of inspection.

2625967 Primary Facility 2017-08-16 Routine/Complete Inspection No Violations Noted The Gesford K3 pad was well vegetated, including the majority of the pad surface and the cut and fill slopes. The access road was solid and stable. There was no bubbling in the well cellars. Agricultural activity was ongoing around the well site with some wrapped round bales being stored on the corner of the site.
