Cabot shut in annuli on well at @9am. A digital pressure gauge was installed on the 7 5/8" x 4 1/2" annulus. Pressure indicated was 13.3 psi. Gauge was removed, and open flow was screened using Model 60 gas meter. Model 60 reading = >5% combustible gas. DEP and Cabot collected samples for isotopic analysis. The pressure gauge was installed on the 9 5/8" x 7" annulus. Pressure reading was 0 psi. Model 60 reading = >5% combustible gas. DEP and Cabot collected samples for isotopic analysis. After completion of sampling, Cabot re-connected vent lines and locked vents open.
2481911 Primary Facility 2016-04-27 Routine/Partial Inspection No Violations Noted The Department was on site at the Greenwood, R. Pad 1 to conduct a Routine Partial Inspection. The E&S Inspection Log indicated that Mike Brilla (TEC) conducted a Post Rain Event Inspection on 4/27/16. It appears as though the landowner has made a road to the left of the access road gate to circumvent said gate. Also, it seems as if the landowner has been working areas on the well site. There is a large area within the well pad surface that looks like wood chips have been strewn across it and sections of plastic packaging are intermixed.

The perimeter ditch does not appear to meet temporary stabilization requirements. The Department requests that the operator investigate the area and deploy a method of erosion control that is consistent with your plan. The production tanks were stained during the inspection. One of the tanks has more staining than the other. The Department requests that the operator inspect the tanks to find the origin of the staining.

2642431 Primary Facility 2017-10-02 Site Restoration No Violations Noted I inspected the site for notice of termination (NOT) on October 2, 2017. The site was surrounded by active agriculture (field corn). The access road leading in was in good shape with no rutting and the site was well vegetated on the cut and fill slopes. The pad surface was solid and stable as well. There were no erosional issues noted.

I recommend APPROVAL of this NOT.

Onsite to initiate 72 hr. pressure build up test "PBUT" on the annuli of the Greenwood 1V, 6H, 7H & 8H wells. All wells, back side of production casing annuli have been isolated and were venting to onsite poly vent tanks. The 6H, 7H & 8H wells (16"x 9 5/8") cemented annuli had open top 16" casings in cellar *. The 1V wells (9 5/8"x 7") cemented annulus was beneath water level in cellar, no bubbling was noted at time of inspection.

- Upon arrival I met with Cabot O&G Reps. Jeremy Rapp and Sully Rutledge. Prior to shutting in wells annuli, initial gas screenings and flow checks were performed at the 1V (7"x 4 1/2") and 6H, 7H & 8H (9 5/8"x 5 1/2") annuli using the Depts. Altair 5X gas meter, calibrated this morning, and Cabots' RS-484 Alicat "Whisper" flow meter. Pre-shut in screenings showed: - 1V: (7"x 4 1/2") = 100% CH4, 9.2 cf/d; (9 5/8"x 7") open casing top in cellar, No bubbling noted. - 6H: (9 5/8"x 5 1/2") = 100% CH4, 15.2 cf
