2532981 Primary Facility 2016-10-31 Site Restoration No Violations Noted Inspection of the Teel C P1 well pad in response to a submitted Notice of Termination request for the Erosion and Sedimentation Control Permit.

The site has been stabilized and vegetation was found to be greater than 70% uniform perennial growth. Temporary E&S BMPs have been removed from the site. No E&S problems were observed at the time of the inspection. The restored pad size is consistent with the restoration plans. Post Construction Stormwater Management BMPs appear to be installed correctly.

Based on the site inspection, I recommend this notice of termination be APPROVED.

2622415 Primary Facility 2017-08-04 Routine/Complete Inspection No Violations Noted I inspected the well pad July 24, 2017. It is a small pad and well vegetated, located on gently sloping ground. There is active agriculture surrounding the site and there were no erosional issues apparent at the time of inspection. The access road was stable as well.

There were no violations noted.
