Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2668472 Primary Facility 2017-12-06 Routine/Complete Inspection No Violations Noted Arrived at 8:45. Weather: sunny, cold. CCI crew onsite changing out the flow arm on the Savage 1H. The 1H and South 1H, wells previously in production, were shut-in. The 4 new conductors are in place. Liner and mats are also in place. Left at 9:10. No violations were observed.
2682013 Primary Facility 2018-01-11 Drilling/Alteration No Violations Noted Arrived at 9:39. Precision 569 drilling the Savage South Unit 2H circulation prior to setting production casing at 14454'. This is the first of 4 new wells planned for this location. Already onsite are the Savage 1H and Savage South Unit 1H.

No violations were observed at the time of my inspection.

2686018 Primary Facility 2018-01-23 Routine/Complete Inspection No Violations Noted I met Ty Marine when I arrived on-site. Precision rig #569 on location, Savage South 3H well drilled to TD (13913') and rig is currently running casing in the hole. Steady rain was falling. Overall, containment was in good condition and functioning properly; however, on the east edge of the sitewide containment, I noted 3 locations where rainwater was overtopping the berm in a steady stream. Ty said he had just had a vac truck working in this area, and another was due shortly. I strongly recommended to Ty that he set up a trash pump or other full-time system to keep this area clear of rainwater. A trash pump was already nearby, and Ty had a tank in the vicinity as well to pump to. Ty was setting up the trash pump when I departed. Based on the concentration of water on the eastern side of the containment, it seems that the pad elevation tapers off in this area, creating a low spot with a high potential for a release; I strongly recommend that Chief either keep all of their primary containment above th
2695796 Primary Facility 2018-02-20 Routine/Complete Inspection No Violations Noted Arrived at 11:00. Weather: warm. Crew from Mr. dirt and Patrik's washing up rig mats at the time of my inspection. There are a total of six wells on this pad. There are two older wells are currently shut in and four new wells they have not been frac'ed yet. No violations were observed the time of inspection.
2709087 Primary Facility 2018-03-23 Drilling/Alteration No Violations Noted Stimulation activities being conducted at this time. No leaks noted during site walk around. Containment appears sound. No violations noted.
2711945 Primary Facility 2018-03-29 Incident- Response to Accident or Event Violation(s) Noted Stimulation activities are occurring at the Savage well pad at this time. On 3/28/18 a truck driver of a residual waste hauling truck incorrectly positioned valving on the manifold system for the residual water (brine) tank battery and inadvertently added fluid to an already full baker tank. This mistake resulted in the baker tank overflowing one barrel beyond (tank position less thank 3 feet from edge of containment) containment.

Clean-up operations were under way during the DEP's visit on 3/29/2018. A vacuum truck and mini excavator were in use to removal material from a 15x30 foot area. All excavated soil was being placed on containment. The material volume to be excavated has yet to be determined. No samples collected by the DEP. Samples to be collected by Woodward and Curran to determine the extent of the clean-up operations. Violations noted. Permit 113-20380 to be referred to the DEP Environmental Clean-ups program for further 78a.66 compliance.

2752434 Primary Facility 2018-07-17 Administrative/File Review No Violations Noted On July 13, 2018 DEP's Environmental Cleanup's and Brownfield (ECB) program approved the state wide health final report pertaining to this release. The violations cited during your March 29, 2018 report ( IR#2711945) or considered resolved.
2799549 Primary Facility 2018-11-06 Routine/Complete Inspection No Violations Noted Chief Oil & Gas submitted a Well Site Restoration Report (WSRR) for the Savage pad. The WSRR was received by the Department on 10/23/18.

Arrive onsite at 12:40; weather was ~50°F, calm, light rain. Some puddles on the pad surface were screened for conductivity and all measurements were below 100 µS/cm.

There were 6 properly tagged Marcellus wells staged for production. All cellars were filled with water; no bubbling was observed. There were 6 GPUs and 6 produced water tanks onsite and in secondary containment. The water in secondary containment around the produced water tanks had a conductivity of 2.50 mS/cm. This water should be removed by vac truck and not be discharged to the ground or permitted to enter Waters of the Commonwealth.

There was a compressor on the pad and in secondary containment. This containment appeared sound but could possibly be better maintained around the edges and corners. There was also what appeared to be ventilation ductwork along the pad (Photo #2). Some sections of compost fi
