2607959 Primary Facility 2017-05-31 Follow-up Inspection Violation(s) Noted Arrived @ 11:30. Weather: overcast. Moran Logistics staff was on site, getting ready for a toe prep on the Nelson Unit 2H and 3H. The purpose of this inspection was to follow-up on Peg Farrell’s 5/26/17 notification of bubbling outside of surface casing for the Nelson Unit 3H. There are 3 wells on this location. At the time of my inspection, the Nelson Unit 1H was producing with compression. The 2H and 3H were TD’ed but not stimulated.

Nelson Unit 3H: The cellar contained water whose surface was approximately 3’ below surface. Bubbles were emerging at a rate of approximately 30 bubbles/minute from 4 locations on the SW side of the GE RSH -2-N wellhead. The bubbles appeared to be emerging from outside of surface casing near the wellhead base. Using tubing and a collection device, I collected sufficient gas bubbling up to detect 3% methane/volume using an Altair 5X gas meter calibrated on 5/31/17. The 13-3/8” x 9-5/8” and 9-5/8” x 5-1/2” annuli were open and venting to a 550-gallon polytank, about a quarter-f

2608056 Primary Facility 2017-06-15 Follow-up Inspection Outstanding Violations - Viols Req'd Arrived @ 09:00. Weather: cloudy, cooler. At the time of my inspection, workers were widening the access road and working on Baker tank connections. Met with Environmental Specialist Jon Hafer (Chief). The purpose of this inspection was to examine the plate welded across the 20” x 13-3/8” annulus, to facilitate monitoring of methane gas outside of surface casing (on the 20” x 13-3/8” annulus) This gas expression was described in IR 2607959, 5/31/17. The GE RSH02-N wellhead sits on top of a landing ring welded to the conductor. To prepare the 20” x 13-3/8” annulus for a welded cap with vent, the landing ring and a segment of the conductor were removed. This exposed the threaded connection between the well head and the 13-3/8” surface casing. Once the portion of 20” conductor was removed, the cement was stripped away from the surface casing and a plate with annulus vent (conductor cap) was attached. 2608070 Primary Facility 2017-06-20 Follow-up Inspection Outstanding Violations - Viols Req'd Arrived @ 11:30 to observe the beginning of a 72-hour Pressure Buildup Test at the Nelson Unit 3H well. All three (3) annulus vents were shut-in at approximately 12:10 on 6/20/2017. Electronic dataloggers were set to collect pressure and temperature data every five minutes throughout the testing period at each annulus vent.

The result of this inspection is OUTST because violations were cited in my 5/31/17 inspection report (IR 2607959), and no new violations were cited during this inspection. Left location @ 12:30.

2614915 Primary Facility 2017-07-11 Follow-up Inspection Outstanding Violations - Viols Req'd : Arrived @ 13:20 to attempt to begin pressure build up test at the Nelson Unit 3H well. Coiled tubing rig was onsite at the time drilling out plugs. As a result, could not begin pressure test.

The result of this inspection is OUTST because violations were cited in my 5/31/17 inspection report (IR 2607959), and no new violations were cited during this inspection. Left location @ 13:45.

2618047 Primary Facility 2017-07-20 Routine/Complete Inspection No Violations Noted Site was in flowback phase, with temporary separators and dehy units connected to the 2H and 3H wells. Two tenders on site with the system. Secondary containments in good condition, and all water on the pad showed low conductance (.37- .65 mS/ cm).
2624631 Primary Facility 2017-08-03 Follow-up Inspection Outstanding Violations - Viols Req'd Weather: cloudy, after rain. Arrived @ 13:00 with Chief Environmental Specialist Jon Hafer to conclude 7-day PBTs on the Nelson Unit 3H. Pressures at the conclusion of the test were as follows:

Nelson Unit 3H: 9-5/8” X 5-1/2”: 409.10 psig @ 13:25; 13-3/8” x 9-5/8”: 12.95 psig @ 13:25, 20” x 13-3/8”: 4.05 psig @ 1325. Made plans with Mr Hafer to isolate, purge, briefly shut-in, and collect isotopic samples on 8/7/17.

2624643 Primary Facility 2017-08-07 Follow-up Inspection Outstanding Violations - Viols Req'd Weather: rain, which stopped during the inspection. Arrived @ 10:41 with Chief Environmental Specialist Jon Hafer to observe isotopic sample collection from annuli on the Nelson Unit 3H.

Nelson Unit 3H, water in cellar: no bubbling. Top gauge and 5-1/2”; 2250 psi. Mr Hafer acquired samples from all three annuli. The result of this inspection is OUTST because violations were cited in my 5/31/17 inspection report (IR 2607959), and no new violations were cited during this inspection. Left location @ 12:02.

2669682 Primary Facility 2017-12-06 Routine/Complete Inspection Outstanding Violations - Viols Req'd Arrived at 12:15. Weather: cold, windy. Met with Jon Hafer (Chief) to observe conclusion of 168- hour PBT and isotopic sample collection.

NElson 3H: 9-5/8" x 5-1/2": 282.05 psig, 13-3/8" x 9-5/8": 5.25 psig, 20" x 13-3/8": 0.75 psig.

2752697 Primary Facility 2018-07-17 Routine/Complete Inspection No Violations Noted Three wells staged for production phase. Three each water separators. Dump lines wrapped in insulating blanket. No leaks detected from production equipment. No violations noted.
2784707 Primary Facility 2018-10-03 Routine/Complete Inspection No Violations Noted Chief Oil &Gas submitted a Well Site Restoration Report (WSRR) for the Nelson pad. The WSRR was received by the Department on 9/11/18.

The pad has been reduced in size and vegetation was well established (>70% cover) at the time of inspection. All PCSM BMPs have been installed and appeared to be functioning as designed.

There are 3 properly tagged Marcellus wells staged for production. There are also 3 produced water tanks, 3 GPUs, and 1 compressor onsite and in secondary containment. There was some minimal corrosion noted on the produced water tanks (Photo #12). There was also corrosion noted around a weld on pipeline leading to the GPUs (Photo #9).

Near the wells there were 2 containers with hoses that were not labeled (Photo #6). A tank of used motor oil was near the compressor with a very faded hazard label(Photo #11). There was also a container in a "Tank Vault" that had no visible label (Photo #7).

It is recommended that all storage containers be visibly and legibly marked with proper labels. It is
