Containment was down and covered with mats. At the time of my inspection an Allison Crane crew was onsite. Also onsite was a Keystone Clearwater Solutions crew. I spoke to Mark Maguire who said that they were putting OBM from the Yonkin B in the tanks on the right hand side (looking toward the road). They were putting water in the tanks at the back on the left, and base oil in the tank at the front on the left.

The Bahl Unit 1H was shut in and surrounded by fencing. Bahl 2H and Bahl 3H conductors were cut and beveled. Left @ 14:00.

2574512 Primary Facility 2017-03-23 Routine/Partial Inspection No Violations Noted Inspection conducted to assess site conditions during drilling operations. I was accompanied my Chief representative Jeff Browning during my inspection. No violations were cited.
2575576 Primary Facility 2017-03-23 Drilling/Alteration No Violations Noted Arrived @ 11:27. Weather: cloudy. Precision 569 on location. Met with company man Jeff Browning. At the time of my inspection he had just set the pack off and was waiting on the intermediate casing cement. Originally he had been drilling the 2H, but due to anti-collision issues with the completed 1H, the 3H was being drilled from the 2H SHL. Intermediate casing was set at 1590’, used 560 sks, 1.253 yield, 17 bbls return to surface (cement report labeled 3H). Surface casing shoe was set at 558’, 535 sks, 20 bbls returned to surface (cement report labeled 2H). For the 2H, they had been originally set to drill at 2° toward the north to avoid the 1H borehole. Due to the lag in MWD tool reporting, didn’t find out until about 200’ that the well was drifting 2° south. Efforts to get back to the drilling program on track did succeed, so at the time of my inspection, Chief was drilling the 3H from the 2H surface location. Jeff Browning said that he would drill the 3H to kick off @ ¿ 7400’.

At the time of my inspection, the rig had completed drilling of the 2H and was back in the 3H finishing the curve. Walked the site with the company man. The location appeared to be well organized. 2606814 Primary Facility 2017-06-15 Follow-up Inspection Outstanding Violations - Viols Req'd Weather: Cloudy, cooler. Arrived @ 14:00 accompanied by Chief Envirnmental Specialist Jon Hafer. The purpose of this inspection was to check on the pressure gauges installed on the 13-3/8” x 9-5/8” annuli of the Bahl 2H and Bahl 3H wells. Bubbling outside of surface casing on these wells was reported by Chief on 6/7/17. At the time of my inspection, the Bahl 2H and 3H were being zipper-fracced. The 5-1/2” x 9-5/8” annuli were linked to the frac plumbing and were monitored for major changes in flow as part of the frac. The 13-3/8” x 9-5/8” annuli were shut-in to continuous pressure monitors. Also in-line was an analog pressure gauge and a pressure relief valve set to release if pressures approached the regulatory limit.

Bahl 3H: 9-5/8” x 5-1/2”: 34 psi, bubbling outside of the surface casing was observed. Bahl 2H: 9-5/8” x 5-1/2”, very low, less than 5 psi, bubbling outside of the surface casing was observed.

Following the inspection of the Bahl 2H and 3H wells, the abandoned well well at: 41.56751, -76.5

2608337 Primary Facility 2017-06-20 Follow-up Inspection Outstanding Violations - Viols Req'd : Weather: warm. Arrived @ 14:00 accompanied by Chief Environmental Specialist Jon Hafer. The purpose of this inspection was to conduct another check on the pressure gauges installed on the 13-3/8” x 9-5/8” annuli of the Bahl 2H and Bahl 3H wells.

The 5-1/2” x 9-5/8” annuli of the Bahl 2H and 3H were linked to the frac plumbing and being monitored by the frac crew for major changes in flow. The 13-3/8” x 9-5/8” annuli were shut-in to continuous pressure monitors. Also in-line was an analog pressure gauge and a pressure relief valve set to release if pressures approached the regulatory limit.

Bahl 3H: 13-3/8” x 9-5/8”: 36 psi, bubbling outside of the surface casing was observed, confirming Chief’s prior notification to the Department.

Bahl 2H: 13-3/8” x 9-5/8”, pressure very low, less than 5 psi, bubbling outside of the surface casing was observed, confirming Chief’s prior notification to the Department.

2608465 Primary Facility 2017-06-27 Routine/Complete Inspection No Violations Noted I arrived on-site @ 09:20 hrs. and met Justin Myers, Chief company man. Frac ended Sunday 6/25, and most equipment had already been moved to the Nelson pad for the upcoming frac there. Several banks of Baker tanks and a pair of pump trucks were still present on the site. Water puddled on the pad surface demonstrated low conductance (.3- .980 mS/ cm). Berms had been removed from pad containment for equipment removal; water puddled on the liner also mostly showed low conductance (.4- 1.3 mS/ cm); however, one area near the pump trucks had puddles ranging from 2 mS/cm to greater than 19.99 mS/ cm. The volume of these puddles were low (the largest being 2-3 gallons), and a vac truck operator was immediately deployed to the area by Mr. Myers. Overall condition of the pad liner appeared to be good. PPC and ER plan were in the site mailbox; I departed the site @ 10:15 hrs.
2614908 Primary Facility 2017-07-11 Follow-up Inspection Outstanding Violations - Viols Req'd Weather: cloudy, threat of rain. Arrived @ 12:10 accompanied by Chief Environmental Specialist Jon Hafer. The purpose of this inspection was to document the start of a PBT test on the 13-3/8” x 9-5/8” and 9-5/8” x 5-1/2” annuli of the Bahl 2H and Bahl 3H wells. The test was motivated by an anomaly in pressure data for the 13-3/8” x 9-5/8” annulus of the Bahl 3H during fraccing operations on the Bahl pad (2H, 3H, 1H). Pressures on this annulus on 6/20/17 increased from 35 psig to 71 psig on 6/25/17 when the test was terminated. At the time of my inspection, preparations were being made to drill out plugs. The 9-5/8” x 5-1/2” annuli were shut in and the 13-3/8” x 9-5/8” annuli were open and venting. Since the 9-5/8” x 5-1/2” annuli were not at open equilibrium prior to the start of the test, we agreed to only gather pressure data only on the 13-3/8” x 9-5/8” annuli. The start time for the Bahl 3H was 12:52.
2616883 Primary Facility 2017-07-17 Follow-up Inspection Outstanding Violations - Viols Req'd Weather: hot. Arrived @ 11:00 accompanied by Chief Environmental Specialist Jon Hafer. The purpose of this inspection was to check on the pressure build for 13-3/8” x 9-5/8” annuli of the Bahl 2H and Bahl 3H wells which began on 7/11/17. At the time of my inspection the three wells on this pad (Bahl 1H, 2H, and 3H) were in production. The 9-5/8” x 5-1/2” annuli were shut-in, but not being monitored. For the Bahl 3H, the bubbling in the 20” x 13-3/8” annulus had increased from the bubbling observed previously. The 13-3/8” x 9-5/8” annulus pressure data showed that pressures rose to approximately 105 psi in approx. 36 hours and then degraded to approx. 74 psi at the time of inspection. As the inspection terminated a crew arrived on site to open the annular vents and plumb them to a polytank.
2624365 Primary Facility 2017-08-03 Follow-up Inspection Outstanding Violations - Viols Req'd Weather: cloudy, after rain. Arrived @ 10:30 to meet Chief Environmental Specialist Jon Hafer to conclude 7-day PBTs on the Bahl Unit 2H and 3H. At the time of my inspection the three wells on this pad (Bahl 1H, 2H, and 3H) were in production. Pressures at the conclusion of the test were as follows:

Bahl Unit 2H: 9-5/8” X 5-1/2”: 590.10 psig @ 12:10; 13-3/8” x 9-5/8”: 9.10 psig @ 12:10, 20” x 13-3/8”: 12.75 psig @ 12:15. 5-1/2” 1600 psi (0-5000 psi analog). Bahl Unit 3H: 9-5/8” X 5-1/2”: 645.60 psig @ 11:15; 13-3/8” x 9-5/8”: 54 psig @ 12:00, 20” x 13-3/8”: 24.55 psig @ 12:05. 5-1/2” : 1800 psi (0-5000 psi analog). Made plans with Mr Hafer to isolate, purge, briefly shut-in, and collect isotopic samples on 8/7/17.

2658596 Primary Facility 2017-11-07 Routine/Complete Inspection Outstanding Violations - Viols Req'd Arrived at 9:50. Weather: cold, cloudt, windy. Andrea Mullen (DEP geologist), Ulyana Trynovich (DEP OGI), and I met with Jon Hafer (Chief Environmental Specialist). There were 3 producing wells on the pad. The purpose of this inspection was to examine the annular plumbing on the 2H and 3H wells (which employ the GE RSH2 well head) and the 1H ( which uses the LSH well head), and participate in the acquisition of production gas isotopic samples for the 2H and 3H wells. Following the well inspections, the abandoned well was monitored for combustible gas. Combustible gas was not detected (Altair 5X gas meter calibrated on 11/7/17). During this inspection a crew was working on one of the Bahl pad sed basins.

2695010 Primary Facility 2018-02-15 Routine/Complete Inspection Outstanding Violations - Viols Req'd Arrived at 12:30. Weather warm with snow and ice melting. There are 3 wells in production on site and an abandoned well off-site, but nearby. Bahl 1H: ice in cellar, 5' below surface, no bubbles. Top gauge: 1200 psi (0-5000 psi). 5-1/2": 1225 psi (0-5000 psi). 9-5/8" x 5-1/2": 36% methane/ volume (Altair 5X gas meter calibrated 2/14/18).

Bahl 3H: cellar water-filled. Top gauge: 1200 psi, 5-1/2": 1150 psi. Bahl 2H: low level water and ice in cellar. Top gauge: 1150, Bahl 2H and 3H are plumbed to the same polytank. Tank about 1/5 full of oil-based mud mixture. 60% methane/ volume at tank mouth. Nearby abandoned well: some water in base of casing - no bubbling. No methane detected.

2785659 Primary Facility 2018-10-09 Routine/Complete Inspection No Violations Noted Weather was 70° and cloudy at time of inspection. There were no chief representatives on location at time of inspection. Well had a 0-5k psi gauge on the production casing and the pay line. Well also had a digital gauge on the production casing. The digital gauge on the production casing read 993 psi and pay line pressure was approximately 1050 psi at time of inspection.There was no gauge on the production tubing. There were 2 annulus vents plumbed to surface. They were plumbed together and ran over to a poly vent tank. The tank had 100% LEL and 8% methane detected at time of inspection. No violations were noted at time of inspection.
2835739 Primary Facility 2019-02-04 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection at 12:38; the weather was ~50°F, clear, and calm. The gate was open when I arrived.

There were 3 properly tagged Marcellus wells staged for production. The well cellars were filled with water; no bubbling was observed. The wells were plumbed to 2 poly vent tanks that were not on additional secondary containment. There were 3 GPU's and produced water tanks onsite and in secondary containment.

There were 2 infiltration basins constructed at this pad. Both basins had some remaining ice in them. Some areas that were free from snow were well vegetated. Sections of filter sock remained in place around the site. The last logged E&S inspection was 1/14/19.

The Department recommends maintaining a proper inspection schedule and noting those inspections in the onsite log.
