2526267 Primary Facility 2016-10-07 Routine/Complete Inspection No Violations Noted Routine inspection conducted to assess progress of site construction and overall conditions. Rough grading on the pad appears to be nearly complete. Perimeter erosion and sediment control BMPs are in place and in good repair. Germination is occuring on areas that have been seeded. No evidence of accelerated erosion or sedimentation was observed.

2527120 Primary Facility 2016-10-12 Preoperation Inspection No Violations Noted Preoperational inspection conducted at the request of Chief Oil & Gas. Keith Kimball and Merle Wade of Chief were present during the inspection.

All erosion and sediment control BMPs are in place as permitted and in good repair. Temporary stabilization is in place on the disturbed areas of the site, and germination of the seed mix was evident over most of the site. Grading and compaction of the final layer of rock ono the well pad surface was nearly complete.

A backhoe with staining on the side and track below the fuel port was parked along the site access road. Upon closer investigation, the staining was observed to be from deisel fuel, and a small area of staining on the ground surface was noted. Judging by the extent of the staining the amount of spilled fuel was less than 5 gallons. Since staining from the spilled fuel was observed on the ground surface, this release had the potential to pollute waters of the Commonwealth, which is in violation of 35 P.S. § 691.402(b). 2557637 Primary Facility 2017-01-25 Routine/Complete Inspection No Violations Noted Arrived @ 11:00. Weather: cloudy, warm, ¿ 3” snow on location. No one was onsite. There were two cellars with conductors in place. Each was surrounded by a cattle fence.

Hurley B 1H, 113-20360, Spud: 1/10/17 Yonkin B 1H, 113-20361, Spud: 1/11/17. It is worth remarking that the wells were spud without notifying the Department prior to the activity. Section 3211(f)(1) of the 2012 Oil and Gas Act provides for 24-hour notification prior to such activity. Discussions with Chief indicate that procedures have been modified to prevent such a lapse in the future

2566545 Primary Facility 2017-02-28 Drilling/Alteration No Violations Noted Arrived @ 09:18. Weather: sunny. Precision 569 rig setting production casing on the Yonkin B 1H. Met with company man Jeremy Boyd. At the time of my inspection rig was at 8445’ with total depth at 14,794. Had planned to go to 15,200 TMD, but hot Marcellus dipped right down so called it a bit short. Progress had been slowed because they had to trip out and replace a bit. BOPs tested on 2/18-2/19, charts looked good. Next well will be the Hurley B 1H on the same pad and then wells on the Bahl. Left@ 10:12.
2569497 Primary Facility 2017-03-09 Routine/Complete Inspection Viol(s) Noted & Immediately Corrected Routine inspection conducted to assess overall site conditions during drilling operations. Ty Marine was the Chief company representative on site. I informed Mr. Marine of the area of sheen observed and recommended that the puddle be removed. Mr. Marine indicated that the puddle would be removed immediately.

I revisited the site on March 08, 2017, with Kellon Balum of Chief. No sheen or odor was observed, indicating that the clean up efforts were effective.

Title 25 Pa. Code Section 91.34(a) requires persons engaged in an activity which includes the impoundment, production, processing, transportation, storage, use, application or disposal of pollutants to take necessary measures to prevent the substances from directly or indirectly reaching waters of this Commonwealth, through accident, carelessness, maliciousness, hazards of weather or from another cause. 2572875 Primary Facility 2017-03-20 Routine/Partial Inspection No Violations Noted Inspection conducted to review site conditions after drill rig moved off site. Containment was in place around the well heads. No evidence of spills was observed in the area outside of the containment. Due to snow cover, erosion and sediment control issues noted during previous inspections were not inspected. No violations were cited.
2577355 Primary Facility 2017-03-31 Routine/Complete Inspection No Violations Noted I arrived on-site@ 14:05 hrs and met Brandon Jessen, Chief company man. Drilling @12,000 ft on synthetic oil based mud. Some minor housekeeping issues noted. In chemical secondary containment, small piles of a white, powdery substance were on the liner. In the cuttings backyard, small amounts of drill cuttings were on the rig mats, with one or two very small spots off the edge of containment. Brandon informed me that a rolloff had just been removed from the backyard, and that he would have both areas cleaned. I departed @14:40 hrs.
2597401 Primary Facility 2017-05-24 Routine/Complete Inspection No Violations Noted I arrived on-site @ 09:25 hrs and met Justin Myers and Floyd Roberts of Chief. Frac completed; Universal Well Services removing frac equipment from site. Coil tubing rig should be arriving on location to begin drilling out plugs this afternoon or tomorrow. Baker tank containments and sitewide liner appear to be in excellent condition, although the berm had been removed for moving the equipment off-site. I departed @10:05 hrs.
2673858 Primary Facility 2017-12-21 Routine/Complete Inspection No Violations Noted Arrived at 1339. Weather: sunny, with temperatures in the 30s. There are two producing wells on this pad the Hurley B 1H had 16 percent methane/volume at the poly tank (Altair five X gas meter calibrated on December 18). The 9 5/8 x 5 1/2" annulus was gurgling. It was not possible to monitor the 13 3/8 x 9 5/8" annulus.

Yonkin B1H: top gauge 1500 psi (0 to 5000 PSI, analog). It was not possible to monitor the anuli on this well for gas.

There were 2 properly tagged Marcellus wells staged for production. The well cellars were filled with gravel. There were 2 GPUs and 2 produced water tanks onsite and in secondary containment. The secondary containment contained a small amount of ice.

The site was greater than 70% vegetative cover and no E&S issues were observed. It appeared that PCSM was addressed. Some filter sock was still in place around the well site.

According to Department records the ESCGP for this pad expires on 2/23/21. A 2-year restoration extension was also acknowledged on 9/12/17.
