At the time of inspection drilling depth was 1540', this will be the set depth for the intermediate casing. Crew from Warrior were preparing to run a CBL on the surface casing. Surface cement job went well with 25 bbls back to surface. No violations at the time of inspection. Left locatoin @ 11:10.

Company man Billy Swanson was at the nearby Hart pad at the time of my inspection. Walked the pad and noted that drilling chemicals were on containment. Left to go to the Hart pad location at 12:30.

Primary and secondary containments were in place in the rig work area. No evidence of spills within the containment was observed. Containment was in place around light plants, generators, and storage tanks situated outside of the rig work area. No evidence of spills within the containment was observed.

2431961 Primary Facility 2015-12-10 Routine/Complete Inspection No Violations Noted Routine inspection conducted to review site conditons after drilling. Crews are on site completing the remainder of rig-down tasks. The well pad surface was visually screened for signs of spills such as staining; no suspect areas were identified. Erosion and sediment controls for the site are in place and in good repair. No evidence of accelerated erosion or sedimentation was observed.
2444103 Primary Facility 2016-01-25 Routine/Complete Inspection No Violations Noted Arrived @ 12:30. Weather: cold, cloudy. There was one TD-ed well on location with a night cap in place and concrete barriers around the well area. There was gravel in the cellar. The top of the gravel was about 2’ below the lip. 17. Annular vents were plumbed into a 500-gallon poly tank. The polytank vent was monitored with an Altair 5X gas meter calibrated on 1/20/16. No methane was detected. There were no violations at this time. Left location @ 13:00.
2464843 Primary Facility 2016-03-15 Routine/Complete Inspection No Violations Noted Equipment is being set up for well stimulation. Containment was in place in the area surrounding the well head and under the sand tanks, pump trucks, and other equipment. Smaller containment units were in place under vehicles and other equipment. Two vaccuum units were in place for use in removing water and fluids from the containment during the stimulation process.

Temporary erosion and sediment controls on site were in good repair. No evidence of accelerated erosion or sedimentation was observed.

No violations were cited during this inspection.

2464885 Primary Facility 2016-03-15 Routine/Complete Inspection No Violations Noted Arrived @ 11:20. Weather: cold, cloudy. Met with company man Justin Myers. The site is being prepared for fraccing. Universal is doing the frac with Cased Hole Solutions handling the wire line. Plan to begin fraccing operations on 3/16/16. Operations should continue for ¿ 5 days. On 3/13/16 did run-in with coiled tubing rig and toe frac. Sand trucks were arriving as I departed the site @ 12:20. No violations were observed at the time of my inspection.
2465850 Primary Facility 2016-03-18 Routine/Complete Inspection No Violations Noted All equipment and chemicals on site were located within containment, and no evidence of spilled materials was observed.

A loose piece of erosion control matting was observed on the berm of a sediment basin off of the southeast side of the well pad. This section of matting should be repaired.

No violations were cited during this inspection.

2466944 Primary Facility 2016-03-22 Routine/Complete Inspection No Violations Noted Routine inspection conducted to review site conditons and status of well stimulation. Crews were preparing to rig up a coil-tube drill rig for removing the plugs.

The electrical conductance of pussles on the pad surface was screened using an Oakton PCtestr handheld probe. The puddle contucctance ranged from 550 - 1,700 umhos/cm.

Appropriate containment was in place under equipment and surrounding the well head.

The loose piece of erosion control matting observed during the March 18, 2016 inspection (Insp. ID. Nos. 2465849 & 2465850) has not yet been repaired.

No violations were cited during this inspection.

2467619 Primary Facility 2016-03-17 Drilling/Alteration No Violations Noted Arrived @ 11:35. Weather: cold, cloudy. Fraccing was in progress at the time of my inspection. Met with company man Brett Fowler, also met Chief Completions Manger Bryce Yeager. Stage 5 was finished during my inspection. The duration for the stage was approximately 2.5 hours. The target parameter was the pump rate (90 bpm). There was a brief shut-down ¿ 1 hour into the frac due to a leak. Otherwise the stage went well. The frac will consist of 19 stages and should be done by Sunday night (3/20/16), according to Fowler. Current estimates for the timing of the flowback are 5-7 months – they are waiting on pipeline. Left location at 13:30.
2467640 Primary Facility 2016-03-21 Drilling/Alteration No Violations Noted Arrived @ 11:15. Weather: sunny. Met with Company man Gib Blevins. Coiled tubing rig was onsite and preparing to drill out plugs following completion of frac job. Pressure at the well head was ¿ 2000 psi, according to Blevins. Also onsite was DEP WQS Chris Yeakel. Took a lap around the location with Yeakel. Left @ 12:15.
2468778 Primary Facility 2016-03-28 Drilling/Alteration No Violations Noted Arrived @ 10:02. Weather: cloudy, following rain. Met with Greg Rohrer (Chief). Flowback began at 9:00, and 1225 psi well head pressure. Flowback is to prove gas and lock in lease, according to Greg. Will flowback at 45-50 bbls/hr until the well produces gas. Then will flare to prove gas. Entire job should take 2 to 5 days. Well head pressure 1125 psi @ 10:30. Also onsite were crew with Oil States. There were no violations at the time of my inspection.
2468825 Primary Facility 2016-03-29 Drilling/Alteration No Violations Noted Arrived @ 10:04. Weather: Sunny, windy. Flowing back,conditions steady since yesterday. Oil States crew onsite were monitoring the flowback, and vaccing out water on containment. Left location @ 10:20. There were no violations observed at the time of my inspection.

Well had been previously flowed back from 3/28/16 for approximately 3 days. Crew members were removing liner and cleaning up. Annular vents were plumbed to a 330-gallon polytank. 2558094 Primary Facility 2017-01-25 Routine/Complete Inspection No Violations Noted Arrived @ 12:40. Weather: cloudy, warm, snow on location. No personnel onsite. The Warburton N 1H is in production. Cellar water-filled, no bubbles.

Top gauge: 900 psi (0-5000 anal) 5-1/2”: 1100 psi (0-5000 anal) Annular vents plumbed to polytank. Left location @ 1:15

2563173 Primary Facility 2017-02-21 Routine/Complete Inspection No Violations Noted No violations cited.
2782342 Primary Facility 2018-09-28 Site Restoration No Violations Noted Chief Oil & Gas submitted a Notice of Termination (NOT) for the Warburton North Unit pad covered under ESG 29-113-15-0005. The NOT was received by the Department on 9/19/18.

There is 1 properly tagged Marcellus well staged for production. There is also one each produced water tank and GPU onsite and in secondary containment.

There are 2 infiltration basins serving as PCSM BMPs. Both appear to have been constructed according to plans and functioning properly. Overall the site is >70% vegetative cover. However, some slopes near the access road are bare (Photos #4, 11).

Due to recent rainfall over the previous days the produced water tank secondary containment was appox. 50% full with water (Photo #9). The GPU secondary containment was almost over topped by rainwater (Photo #10).

The department recommends returning the NOT and resubmitting after the areas of the pad near the access road have been reseeded and vegetation is better established.
