2451640 Primary Facility 2016-02-18 Routine/Complete Inspection No Violations Noted Routine inspection conducted to assess general conditions on site. No Chief representatives were present.

Site vegetated with >70% vegetative cover, and temporary erosion and sediment control BMPs have been removed. No ESCGP was issued for this site as the total area of disturbance is less than 5 acres. No evidence of accelerated erosion or sedimentation was observed.

Drilling is currently underway. Containment was in place under the drill rig and appurtenant equipment. Containment was also being used for pumps and generators that were not within the drilling mat area. No evidence of leaks or spills was observed. At the time of inspection, the pad surface was very muddy. However, all of the mud was contained on the pad.

No voilations were cited during this inspection.

2460842 Primary Facility 2016-03-02 Routine/Complete Inspection No Violations Noted Routine inspection conducted to assess general conditions on site. No Chief representatives were present.

Drilling activity is still underway. Site containment was in place and all equipment, chemicals, and fuels on site were located within the containment. No evidence of spills was observed outside of the containment on the pad surface. Some containments were partially filled with water; the water should be removed to provide maximum containment capacity.

No violations were cited during this inspection.

2462527 Primary Facility 2016-03-07 Drilling/Alteration No Violations Noted Patterson 254 onsite, met with Company Man Paul Rico. Drilling depth was 685’for the Signore North Unit 5H at the time of my inspection. Drilling was stopped following BOP testing. Surface cement job had 14 bbls returned to surface. Drilling the surface hole had damaged the bit.

The rig previously drilled the Signore North Unit 4H. While pulling out of the well bore, the BHA washed out and had been left at the base of the curve. BHA had been fished out. Also on the 4H, mud weight had been increased to 13.1 ppg due to gas in the lateral. Left location @ 13:20. There were no violations at the time of my inspection.

2529570 Primary Facility 2016-10-20 Routine/Complete Inspection No Violations Noted Routine inspection to assess site conditions during flowback operations.

Screened electrical conductivity in puddls on the well pad surface. Readings ranged from 242 uS/cm to 879 uS/cm. No evidence of any spills such as staining, sheens, or odors were observed.

An area of erosion has started near the southeast corner of the well pad. This appears to be the low corner of the pad, and it appears the flow has concentrated and eroded pad material down the fill slope. None of the eroded material has left the limit-of-disturbance for the site. Merle Wade of Chief was on site and informed of this finding.

No violations were cited during this inspection.

2534932 Primary Facility 2016-11-02 Routine/Complete Inspection No Violations Noted Arrived @ 11:17. Routine inspection of unconventional gas pad following stimulation and flow back. The site was wet following heavy rain, a worker onsite was vaccing up standing water. Flow back equipment was onsite. The Signore North Unit 4H and Signore North Unit 5H were in production. The 1H was not in production at the time of my inspection. Left location @ 11:30.
2598750 Primary Facility 2017-05-26 Routine/Complete Inspection No Violations Noted Three wells in production phase. Three separators within containment. No leaks from production equipment. Site has very steep fill slopes and requires further stabilization to prevent further erosion.
2812133 Primary Facility 2018-12-06 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection at 12:57; the weather was ~30°F and windy.

There were 3 properly tagged Marcellus wells staged for production. The well cellars were filled with ice. There were 3 GPUs onsite and in secondary containment. The secondary containment onsite was about 25% filled with ice.

The pad area was reduced in size and the site was greater than 70% vegetative cover. No E&S issues were observed.

No violations noted at the time of inspection.
