2464896 Primary Facility 2016-03-16 Site Restoration No Violations Noted Interim restoration has been implemented on the well pad. A significant portion of the pad surface has been decompacted and vegetated. While not required due to permit issuance prior to November 19, 2010, an infiltration basin is present off the northeast corner of the pad. A total of 419.72 tons of drill cuttings, 569.32 barrels of drilling fluid, and 439.48 barrels fracing fluid are reported to have been disposed of at appropriate waste disposal facilities. The reported amounts were divided equally amongst the four wells.

The site was determined to meet the requirements of 25 Pa Code §102.22(a)(2) (i) Permanent Stabilization. A minimum uniform 70% perennial vegetative cover, with a density capable of resisting accelerated erosion and sedimentation was observed. All temporary erosion and sedimentation BMPs were removed. No violations were cited during this inspection.

Based on the conditions observed during this inspection, this permit is recommended for termination.

2519598 Primary Facility 2016-09-16 Routine/Complete Inspection No Violations Noted Routine inspection to assess site conditions during well completion activities. Well completion activities have stopped, and crews are breaking down the euqipment. All equipment and chemicals on site were within containment, and signs of spills to containment were observed. Further, no evidence of spills on the pad surface such as staining or odors were observed.

No violations were cited during this inspection.

2525830 Primary Facility 2016-10-06 Routine/Complete Inspection Pending Routine inspection to assess site conditions after well completions. A coil-tubing rig was on site, and work was underway to connect the wells to the separator units and pipeline.

Overall site conditions were good. All equipement and chemicals on site were within containment. The electrical conductivity of several puddles on the well pad surface was screened and generally ranged from 700 - 1,000 uS/cm. However, one puddle located adjacent to the containment for the produced water tanks was screened at 5.2 mS/cm.

The puddle with the elevated conductivity was approximately 3 feet wide and 10 feet long. A piece of discarded containment was in the puddle. I informed the on-site Chief representative, Gil, of my findings and recommended that the puddle be removed. A sample of this puddle was taken for SAC 942 analysis.

The elevated conductivity in this puddle could be indicative of a release of produced water, flowback fluids, or frac fluids. A release of such fluids to the pad or ground surface would consti

2539099 Primary Facility 2016-11-22 Routine/Complete Inspection No Violations Noted Arrived @ 12:00. Weather: cold, windy, clouds. Chief environmental specialist Aaron Stredny was onsite checking puddle conductivites at the time of my inspection. Wells that had been fracced ¿ 2 months ago were in production and pad liner/containment had recently been removed. Two banks of Baker tanks were onsite. Stredney said that the plan was to store brine prior to the Blanchard frac, scheduled for December 9. There were 5 wells in production on location. Annular vents were plumbed to polytanks.

Temporary erosion and sediment control BMPs are in place and are in good repair. Permanent stabilization has been achieved over most of the site, with the exceptions being the soil stockpiles created when gravel was replaced on the pad surface. Those stockpile areas have been stabilized with annual vegetation. No evidence of accelerated erosion or sedimentation was observed.

2735988 Primary Facility 2018-05-31 Routine/Complete Inspection Viol(s) Noted & Immediately Corrected A T-fitting failed on a separator which resulted in 20 gallons of brine to be released to the pad surface. The operator had a vacuum truck on location the night of the failure for clean-up. The next day, on 5/31 the operator began excavating affected soil and rock on the pad surface. The removed material was placed within roll-off containers. Violation noted but immediately corrected.
