Vegetative cover on site is greater than 70% in some areas, but less in others. Temporary erosion and sediment control BMPs are in place and in good repair. No evidence of accelerated erosion or sedimentation was observed.

According to the Department's records, a request for a 2-year extension of time for site restoration was received on 03/16/2016. As of the date of my inspection, this request has not yet been approved.

No violations were cited during this inspection.

2556813 Primary Facility 2017-01-30 Routine/Complete Inspection No Violations Noted Routine inspection to assess overall site conditions. No Chief representatives were present during the inspection.

Vegetative cover on site is greater than 70% in most places though some areas were obscured by snow cover, and temporary erosion and sediment control BMPs are in place. No evidence of accelerated erosion or sedimentation was observed.

This site was due for restoration in November 2015. According to the restoration schedule submitted by Chief, this site is anticipated to be restored during Spring 2018. While this proposed schedule has been acknowledged by the Department, Chief should still submit a request for a 2-year extension of time to restore the site in accordance with Title 58 Pa. C.S. § 3216 (relating to site restoration) (2012 Oil and Gas Act).

No violations were cited during this inspection.

2682068 Primary Facility 2018-01-12 Routine/Complete Inspection No Violations Noted Three wells in production phase. Three each sand and water separators within containment. Natural gas powered suction engine on location. No leaks detected from production equipment. No violations noted.
2811593 Primary Facility 2018-12-04 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection at 12:21; the weather was ~30°F, and calm.

There were 3 properly tagged Marcellus wells staged for production. All the well cellars were filled in with gravel. There were 3 GPUs, 3 produced water tanks, 1 compressor, and 1 freshwater tank onsite.

Two infiltration basins were onsite as PCSM. Both basins contained a fair amount of ice and snow. The rest of the area was free from snow and was greater than 70% vegetative cover. No E&S issues were observed at the time of inspection.

According to Department records the ESCGP for this pad expires on 3/23/19. It also appears that a 2-year site restoration extension was acknowledged on 10/5/18.

No violations were noted at the time of inspection.
