Vegetative cover on site is generally greater than 70%, with rocky areas comprising most of the unvegetated areas. Temporary erosion and sediment control BMPs have been removed. No evidence of accelerated erosion or sedimentation was observed.

No puddles were present on the pad surface at the time of inspection, so electrical conductivity was not screened. No staining, odors or other evidence of potential spills was noted. The well cellars were observed to be nearly filled with fluid and should be emptied.

Currently no active well permits exist for the site. Chief filed a request for an extension of time for site restoration on 02/16/2016 under Title 58 Pa. C.S. § 3216 (2012 Oil and Gas Act)(relating to restoration). That request has not yet been approved by the Department.

2799461 Primary Facility 2018-11-06 Routine/Complete Inspection No Violations Noted Arrived onsite at 11:13 for a routine inspection; the weather ~50°F, raining, breezy. Several larges puddles on the pad were screened for conductivity and all readings were less than 75 µS/cm.

There were 2 properly tagged Marcellus wells staged for production. Both well cellars were filled with water; no bubbling was observed but rain was disturbing the water surface as well. There were 2 GPUs 2 produced water tanks, and 1 compressor onsite and in secondary containment. The water in GPU secondary containment had a slight sheen that appeared to be oil based (Photo #3).

The pad remained at its original size an all PCSM appeared to be installed and functioning as designed. Site restoration plans onsite indicate that the pad would be reduced in size and some areas regraded. It appears that the ESCGP for this site expires on 4/15/19 and any earth moving activities should be completed before that date. Additionally, a 2-year site restoration extension was granted until 10/5/20.
