While this proposed schedule has been acknowledged by the Department, Chief should still submit a request for a 2-year extension of time to restore the site in accordance with Title 58 Pa. C.S. § 3216 (relating to site restoration) (2012 Oil and Gas Act).

2650947 Primary Facility 2017-10-23 Routine/Complete Inspection No Violations Noted Arrived at 14:58. Weather: overcast, windy. One well capable of production onsite, not in production at the time of my inspection. No violations observed.

2791568 Primary Facility 2018-10-18 Routine/Complete Inspection No Violations Noted Chief Oil & Gas submitted a Notice of Termination (NOT) for the PA Woodlands Unit Pad covered under ESG29113140008. The NOT was received by the Department on 10/9/18.

Arrived onsite at 10:03; the weather was ~35 °F, sunny, windy. A few puddles of rainwater remained on the pad but were too shallow for conductivity screening.

There was 1 properly tagged Marcellus well onsite. The cellar was filled with water; no bubbling was observed. There was also 1 GPU and 1 steel produced water tank both in secondary containment. Conductivity of water in each secondary containment was 739.0 µS/cm and 195.0 µS/cm respectively. There was minor corrosion on the produced water tank.

The site is >70% vegetative cover and all PCSM has been constructed as depicted on restoration plans. An orange safety fence remains onsite (Photo #5). The stone portion of the pad appears to remain at the original size. No E&S issues were observed.

It is recommended that the fence be removed if it is no longer necessary. It is also recommended
