Elevated conductance was observed in all puddles on the pad (2.8-6.8 mS/cm) and the sediment basin (2.92 mS/cm) adjacent to the northeast corner of the well pad. No stressed or dead vegetation was observed surrounding the pad, between the pad and sediment basin, or downslope of the sediment basin.

The electrical conductance of fluid puddles within the containment surrounding the well heads was over the instrument reading (20 mS/cm) limit of my Oakton Multi-parameter PCSTestr™ 35. The amount of fluid present within the containment should be monitored and removed as necessary. No areas of staining on the pad surface were observed.

2490926 Primary Facility 2016-06-01 Routine/Complete Inspection No Violations Noted Vegetative cover on site is less than 70%. Temporary erosion and sediment control BMPs are in place and in good repair. No evidence of accelerated erosion or sediment control was observed.

Approximately 30 brine water tanks are located within containment on site. A number of the tanks are full , and tanker trucks are regularly arriving to empty the tanks. No evidence of spills such as staining, elevetated electrical conductivity, or salt crust/crystals was observed.

This site is currently not compliant with the Department's regulations pertaining to site restoration (Title 58 Pa. C.S. § 3216). The Department's records indicate that a request for a 2-year extension of time for site restoration was filed on 02/16/2016; this request has not yet been approved.

No violations were cited during this inspection.

2817842 Primary Facility 2018-12-18 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection with Jared Jacobini, DEP Water Quality Specialist, at 14:24; the weather was ~30°F.

There were 4 properly tagged Marcellus wells staged for production. There were 4 GPUs, 4 produced water tanks and 1 fresh water injection tank onsite and 1 compressor in secondary containment. The GPU and produced water tank secondary containment was about half full with ice. The produced water tanks had some moderate pitting/corrosion (Photo #4). The labels on the water injection tank were faded and illegible.

On the east side of the pad there was a large area of erosion in the collection ditch leading to an infiltration basin (Photos #6, 7). It is recommended that the area be stabilized/restored as per Department guidelines. There is also a swale entering the opposite side of the infiltration basin that had some ice. This area may need attention to ensure that it is functioning as intended.
