Vegetative cover on site is greater than 70%. It appears that post-construction stormwater management features were recently installed. No evidence of accelerated erosion or sedimentation was observed. The electrical conductance of puddles on the well pad was not screened due to the absence of puddles.

A 2-year extension for site restoration was approved by the Department on March, 03, 2016.

There were two wells on a three-slot set-up. The third slot was covered by a steel plate. Both the Brule N Sul 2H and the Brule S Sul 2H were on Regulatory Inactive Status and were shut, locked, and tagged. Both cellars were overflowing with water, no bubbles. Annuli were not accessible for monitoring. Left location @ 12:00.

There were 2 properly tagged Marcellus wells and 3 well cellars onsite. The cellars were filled with ice. There was secondary containment that appeared to be for produced water tanks. The containment was completely filled with ice (Photo #2). There were no storage facilities or production equipment onsite.

Overall, the area was greater than 70% vegetative cover. No E&S issues were observed.

There were 2 infiltration areas on the south side of the pad. These areas were very full of water and ice at the time of inspection (Photo #1). It may be possible that the volume of water is due to higher levels of ground water and partially frozen soil.

It is recommended that PCSM BMPs be monitored and maintained as needed. It may be necessary in the spring to perform maintenance or add an underground drain to ensure the proper function of PCSM.
