At the 13-3/8” x 9-5/8” annulus there was 5% methane. I could not obtain a separate fiend reading at the 9-5/8” x 5-1/2” annulus. There was some staining immediately around the well cellar. The other well, the 7V, was capped. The cellar was partly water-filled with an oily-looking scum on top. Field readings were 0% methane for both the 13-3/8” x 8-5/8” annulus and 9-5/8” vent. Met Kevin Hadlock towards the end of my inspection. Left @12:30
2481105 Primary Facility 2016-05-04 Routine/Complete Inspection No Violations Noted The site currently has >70% coverage of perennial vegetation; temporary erosion and sediment control BMPs are still in place. No evidence of accelerated erosion or sedimentation was observed.

The electrical conductance of puddles was not screened due to the absence of puddles.

The Erosion and Sediment Control General Permit for this site expires on 06/17/2016. No current well permits exist. Chief has indicated that it plans to partially restore the well pad sometime in 2016. In order to remain in compliance with the Department's regulations regarding site restoration (Title 58 Pa. C.S. § 3216), Chief should obtain a new drilling permit or request a 2-year extension of time for site restoration.

No violations were cited during this inspection.

2513652 Primary Facility 2016-08-04 Routine/Complete Inspection Violation(s) Noted Arrived @ 10:30. Weather: sunny, warm. No company representatives were present during the inspection. There were 2wells and a small compressor on the site. The Kuziak 1H was in production with artificial lift. This follow-up inspection was prompted by an Inactive Well Status application for Kuziak Unit 7V. Previously on 6/30/16, combustible gas was detected on the Kuziak Unit 1H well at the 13-3/8” x 9-5/8” annulus. Typically, the 1” , 13-3/8” x 9-5/8” annular vent T’s into the 2” 13-3/8” x 9-5/8” annular vent, allowing for cross-contamination. To eliminate that possibility in this follow-up inspection, the annuli were open and isolated from each other prior to my arrival on site. Monitoring was conducted with Altair 5X gas meter (calibrated: 7/26/16).

Kuziak 1H .13-3/8” x 9-5/8”: 17%/volume combustible gas, 9-5/8” x 5-1/2”: 9%/volume combustible gas. The combustible gas detected at the 13-3/8” x 9-5/8” annuli of the Kuziak 1H well is evidence of defective cement which is a violation of Title 25 §78.8

2556777 Primary Facility 2016-01-30 Routine/Complete Inspection No Violations Noted Routine inspection conducted to assess general conditions on site. No Chief representatives were present.

This site has been partially restored. The pad has been reduced in size, and much of the remaining area has been de-compacted and vegetated. Temporary erosion and sediment controls were in place around the perimeter of the site. No evidence of accelerated erosion or sedimentation was observed.

2788176 Primary Facility 2018-10-11 Routine/Complete Inspection No Violations Noted Chief Oil & Gas submitted a Notice of Termination (NOT) for the Kuziak pad covered under ESG29-113-18-0001. The NOT was received by the Department on 10/5/18.

It had rained throughout the day and during part of the inspections. Some small puddles had formed on the pad and had a conductivity below 100 µS/cm. The pad has been reduced in size and the majority is now vegetation. The areas of vegetation around the site were >70% cover. No E&S issues were observed.

There is 1 active well and 1 inactive well onsite. The well cellars were filled with water; no bubbling was observed. There is also 1 GPU, 1 compressor, and 2 steel produced water tanks onsite and in secondary containment. All containment appeared to be in good condition.

The PCSM measures consist of 1 infiltration berm near the southwest corner of the pad. The berm appeared to be installed and functioning as designed.

No violations noted.

The Department recommends acknowledging termination of coverage under ESG29-113-18-0001.
