2453199 Primary Facility 2016-02-25 Routine/Complete Inspection No Violations Noted Routine inspection conducted to assess general site conditions. No Chesapeake representatives were present.

Vegetative cover on site is greater than 70%, and temporary erosion and sediment control BMPs have been removed. No evidence of accelerated erosion or sedimentation was observed.

No violations were cited during this inspection.

2502451 Primary Facility 2016-07-15 Routine/Partial Inspection No Violations Noted Partial inspection conducted to determine the status of the post construction stormwater management (PCSM) BMPs required by approval of extension of time for site restoration.

The extension of time for site restoration (approved 02/2016), required that the PCSM BMPs be installed within three months. The approved PCSM BMPs have been constructed as approved. Temporary stabilization and erosion and sediment control BMPs are in place.

No violations were cited during this inspection.

2811663 Primary Facility 2018-12-06 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection at 13:38; the weather was ~30°F and calm. A Chief employee was onsite working on the well when I arrived.

There was 1 properly tagged Marcellus well staged for production. There was 1 GPU and 1 sand separator onsite and in secondary containment. The produced water tank containment was about 25% full with ice.

One corner of the pad surface was restored to meadow. The areas around the pad were greater than 70% vegetative cover. Overall, the site appeared stable.

No violations noted.
