2479684 Primary Facility 2016-04-29 Routine/Complete Inspection No Violations Noted Routine inspection conducted to assess general site conditions. No Chesapeake representatives were present.

Vegetative cover on site is greater than 70%, and temporary erosion and sediment control BMPs are still inplace. No evidence of accelerated erosion or sedimentation was observed. The electrical conductance of puddles on the well pad was not screened due to the absence of puddles.

A request for a 2-year extension for site restoration was received by the Department on March, 22, 2016.

No violations were cited during this inspection.

2554504 Primary Facility 2017-01-23 Routine/Complete Inspection No Violations Noted Routine inspection conducted to assess overall site conditions. No Chesapeake representatives were present.

Vegetative cover on site is greater than 70%, and temporary erosion and sediment control BMPs are still in place. No evidence of accelerated erosion or sedimentation was observed. The electrical conductance of puddles on the well pad was not screened due to the absence of puddles.

A request for a 2-year extension for site restoration was received by the Department on March, 22, 2016. To date the extension has not been approved.

No violations were cited during this inspection.

2596547 Primary Facility 2017-05-19 Routine/Complete Inspection No Violations Noted One well in production phase. One each sand and water separator. At least the texting from production equipment. Site stable. No violations noted.
2727498 Primary Facility 2018-05-08 Complaint Inspection No Violations Noted Complaint received due to brown vegetation near the Benspond well site. Complainant was concerned of a spill from the well site.

From Department Complaint Tracking System "Complaint rec'd by Chris Mileto (WW): Complainant states one of his inspectors took photos of a Chief Oil & Gas well pad because he noticed the grass turned yellow around the well site. Complainant not sure if something is leaking."

One well staged for production phase. One each sand and water separator. Puddle on well pad tested at 0.10 mS/cm. No leaks detected from production equipment. No violations noted.

2798108 Primary Facility 2018-11-02 Routine/Complete Inspection No Violations Noted Chief Oil & Gas submitted a Notice of Termination (NOT) for the Benspond Unit pad covered under ESX10-113-0016. The NOT was received by the Department on 10/29/18.

Arrived onsite at 13:59; the weather was ~55°F and breezy. Due to rainfall there were several large puddles on the pad surface. Several locations were screened for conductivity and all measurements were below 150 µS/cm.

There was 1 well staged for production with a total of 6 cellars onsite. All cellars were filled with water; no bubbling was observed. There was 1 each GPU and sand separator and 2 produced water tanks all in secondary containment. There was some rust staining on the produced water tanks.

Overall, the site was >70% vegetative cover and appeared stable. However, there was a path over the containment berm and down the N side of the pad (Photos #4, 5). This path appeared to continue into a neighboring field. There was also compost filter sock (CFS) left near infiltration areas 1 and 2 (Photo #3). On the pad itself there were some s
