2471530 Primary Facility 2016-04-05 Routine/Complete Inspection No Violations Noted Routine inspection conducted to assess general site conditions. No Chesapeake representatives were present.

Vegetative cover on site is greater than 70%, and temporary erosion and sediment control BMPs have been removed. No evidence of accelerated erosion or sedimentation was observed. The electrical conductance of puddles on the well pad ranged from 220 uS/cm to 470 uS/cm.

The Department's records do not show that a request for an 2-year restoration extension has been requested for this site. In the absence of an active drilling permit, the site must either be restored or an extension requested.

2540597 Primary Facility 2016-12-01 Routine/Complete Inspection No Violations Noted Routine inspection conducted to assess general site conditions. No Chesapeake representatives were present.

Vegetative cover on site is greater than 70%, and temporary erosion and sediment control BMPs have been removed. No evidence of accelerated erosion or sedimentation was observed.

2719447 Primary Facility 2018-04-18 Routine/Complete Inspection No Violations Noted Flow-back operations were in process during the DEP's inspection. Two wells in flow-back with sand separators installed. Several conductivity checks were performed on the well pad surface as well as in containment and no evidence of a suspected spill or leak was detected. Baker tank containment near open hatch of one baker tank. Operator has history (see inspection report 2702013 and 2711945) of overflowing baker tanks releasing beyond containment. Larger surface area of containment recommended around tanks. No violations noted. No enforcement recommended at this time.
2733580 Primary Facility 2018-05-22 Routine/Complete Inspection No Violations Noted Arrived at 12:45. Weather: rain. This is a very messy site with garbage strewn at the corner of the pad. Cellars were overflowing water with surface sheen at the time of my inspection. Waste rags, lubricant cans and other containers were on the ground and in the cellar water (see photos). Called Peg Farrell who indicated that the condition of the location would be promptly addressed. There were 2 producing wells on location.
2827038 Primary Facility 2019-01-14 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection at 13:27; the weather was ~30°F and calm.

There were 2 properly tagged Marcellus wells staged for production. The well cellars were filled with water and partially frozen; no bubbling was observed. The well annuli were plumbed to a poly tank not on any additional secondary containment. There were 2 GPUs and produced water tanks onsite and in secondary containment.

The majority of the site was greater than 70% vegetative cover except for the areas noted (Photos #1-3). These areas by Campbellville Rd. lacked sufficient vegetation and showed rills and a small slip.

There was a small amount of equipment on the pad that appeared to be older and possibly not needed (Photos #4-6).

It is recommended that, if the pad meets restoration criteria, an NOT be submitted for review in the spring or summer.

No violations noted.
