2743245 Primary Facility 2018-06-15 Plugging(Includes Plugged/Mined Through) Violation(s) Noted The operator began plugging operations on this well noting that there was no pressure on the well on either the 2 3/8" production tubing or the outside casing when plugging operations commenced. No BOP and no stripper head were installed on this well prior to plugging for well control purposes. After pulling 80 joints of the 8039' string of 2 3/8" tubing from the well, the well started spitting fluid at 11:30am on Thursday, June 14th according to the Site Supervisor. The crew hooked up a trash pump and hose from the plastic lined pit to the tank and another hose from the tubing to the tank also according to the Site Supervisor. Gas showed up at the surface at 12:15 and the well began flowing fluid at 12:37. The crew then shut down and the Site Supervisor arrived on site at 12:57. The Site Supervisor informed me that he then called DEP Inspector Paul Wyrwas at 12:58 to inform him that the well was flowing back and he got his phone out and verified this by showing me the call that he made to the Inspector whic
2743561 Primary Facility 2018-06-15 Administrative/File Review In Compliance with Policy Inspection conducted in response to operator report of a release of gas/brine during plugging operations. Release was reported to DEP Duty officer at 16:30 on 6-14-18. DEP ERT member Tom Buchan responded to this incident at 18:30 on 6-14-18.

This inspection documents on-site interview of operator representatives on 6-15-18. According to Josh Williams, gas and brine was released from the well at approximately 12:15 on 6-14-18. Mr. Williams states that the equipment was shut down at that time but the release continued. Mr. Williams also states that when the gas/brine was released, brine was sprayed over the equipment/site and brine was spread by the wind away from the well head. In addition, Mr. Williams stated that when the plugging equipment was shut down, the well cellar overflowed. Mr. Williams stated no fluids from the well cellar left the site; a small amount of fluid overflowed one corner of the cellar. At time of inspection E and S plan is on site. Temporary BMP's installed per the E and S plan. No E and S issues observed on access road. At this time, plugging operations have ceased and well appears shut in. No active release observed at this time. Plugging rig located on plastic liner (photo #1). Fluid tank and brine tank located on secondary containment (photos 2-3). Well cellar lined with plastic (photo #4). A small, dry, gully is present on the south east corner of the well pad (photo #5). A white, crystal-like substance is present on some of the rocks in this gully (photo #6). This gully appears to be a collection point for rainwater leaving the site. This gully feeds an unnamed tributary to Cranberry Glade Run. Due to the apparent tendency of fluids to channel here, the Department recommends operator install additional BMP's above this gully to ensure any rainwater that falls

As requested by Department (see inspection record #2743606) operator rep Vanessa Crayne submitted photo via email (photo #1) showing temporary BMP installed up-gradient of gully. Violations issued on 6-15-18 remain outstanding. A follow up inspection will be conducted at a later date.

2745459 Primary Facility 2018-06-19 Follow-up Inspection Outstanding Violations - Viols Req'd Follow up Inspection to inspection record #2743606 where violations were issued for failure to report a release to the Department within 2 hours of occurrence, failure to collect brine or other fluids, and failure to properly store, transport, process or dispose of a residual waste.

Operator rep Vanessa Crayne present during inspection. Ms. Crayne states that clean-up contractor began removing impacted soils at 07:30 on 6-18-18 and that clean up was completed at 06:00 on 6-19-18. Ms. Crayne states that 23 truckloads of soil were removed from site and 1 drum of fluid that was pumped from the well cellar. At time of inspection rock has been added to sections of the access road. It appears soil has been removed from well pad (photo #1). Ms. Crayne states well pad will be constructed today and tomorrow and plugging equipment moved on site Thursday. It appears that impacted soil has been removed from gully on south east corner of pad and rock has been added to the gully (photo #2). It does not appear that any so

2745891 Primary Facility 2018-06-25 Plugging(Includes Plugged/Mined Through) Pending On this date the plugging rig was rigged up to circulate bottoms up to clear salt build up out and trip the 2 3/8" pipe out of the hole. The crew was waiting on the pump truck to arrive at the time of my inspection.
2745895 Primary Facility 2018-06-26 Plugging(Includes Plugged/Mined Through) Pending On this date, the plugging rig ran a gauge ring to bottom TD at 8060' to determine that the CIBP could be run to the depth where it needed to be. The CIBP was then run to 8020' and set. At the time of this inspection the logging truck was logging the hole. TOC was established at 7202' and they were still logging up hole when I had to leave. The forward plan is to cut the 4 1/2" casing at the cement top and attempt to pull it. If the rig cannot pull it the operator is going to use casing jacks to assist.

E and S plan on site. At time of inspection BMP's near bridge over Cranberry Glen Run appear to have failed. Compost Filter sock and rock filter on south side of access road do not appear to be functioning as intended (photo #1). Hay bale on north side of access road is not on E and S plan and does not appear to be functioning as intended (photo #2). These BMP's are located on the western side of Cranberry Glade Run and are allowing sediment to enter Cranberry Glade Run (photos #3-4). Department contacted operator rep Corey Giles by phone from site. Mr. Giles stated someone would be sent to site and will follow up with Department. 2746655 Primary Facility 2018-06-28 Plugging(Includes Plugged/Mined Through) Pending On this date the plugging rig ran back in to check the TOC. The last cement plug had been set at 7220' and has now dropped back to 7380'. The cement crew was arriving on site at the time of this inspection to cement again back up to 7200'. The forward plan is to cement and shut down, wait on cement, run back in to make sure cement is up to 7200', shut down for the plugging contractors 4th of July company wide week of vacation and return to this job on Monday July 9th to cut the 4 1/2" at the TOC and attempt to pull the pipe and continue with the plug job. Should the 4 1/2" pipe not pull they will run a free point to see where the 4 1/2" pipe is free to and cut/pull and perf and squeeze cement if necessary. All equipment on site looked ok.
2746803 Primary Facility 2018-06-28 Follow-up Inspection Outstanding Violations - Viols Req'd Follow up inspection to inspection record #2746290 where violations were issued for industrial waste discharged into Waters of the Commonwealth, for industrial waste discharged into the Waters of the Commonwealth without a permit, authorization, or contrary to rules and regulations of the Department, and for failure to implement and maintain E and S BMPs to minimize the potential for accelerated erosion and sedimentation.

At time of inspection, no sediment is entering Cranberry Glade Run (photo #1-2). A rock filter has been added up gradient of the hay bale (photo #3). In addition, it appears additional rock has been added to the rock filter near the compost sock (photo #4). Department contacted operator rep Jeff Osborn by phone. Mr. Osborn states rock filter was added near hay bale and the rock filter near the compost sock was improved. Also, Mr. Osborn states several areas along the access road and near the well pad were reseeded and mulched. Mr. Osborn clarified that he drew the red-line changes on the E

2750001 Primary Facility 2018-07-09 Plugging(Includes Plugged/Mined Through) Pending On this date the plugging rig ran in the 2 3/8" tubing to the TOC, attempted to shoot the tubing off but the tools loaded up with water and the attempt was unsuccessful so the crew tripped out of the hole, cleaned all the water out, ran back in and made the attempt to shoot the tubing off. The attempt was not successful so they tripped back out of the hole. I received a call later in the evening from the Conserv Rig boss Ed telling me that they apparently made a mistake by not pulling on the tubing as it was shot to pull it free. Ed said the rig was idling in neutral when the attempt to utilize the tool from Wyoming which they are not familiar with was made to part the tubing and that they will attempt to re-shoot the tubing again this morning while pulling on it with 50,000 psi to see if it will come free this time.

2752322 Primary Facility 2018-07-16 Plugging(Includes Plugged/Mined Through) Pending The 2 3/8" tubing came free down hole and the rig began pulling stands. When the collar at the top shot of 5620' surfaced it was noted that the shot had ripped a straight 9" slit through the tubing right below the collar and right to the very bottom of the collar without touching the collar. So the tubing had not come free at that point. The rig continued pulling tubing until the next shot at 5650' was exposed. This had been a primer cord shot and while it swelled the pipe above and below the collar and swelled the collar some, it had not parted the pipe. So the rig continued pulling stands of tubing until the last joint of pipe came out of the hole from 6227' down which was the location of the lowest and original shot. It was noted that this shot had split the pipe pretty good right through the threads which came free from the collar. The forward plan at this point was to plumb in a choke to bleed of the 8 5/8" casing which I was informed had 1200psi on it and then run in to tag the cement top in preparatio
2753042 Primary Facility 2018-07-17 Plugging(Includes Plugged/Mined Through) Pending On this date the 8 5/8" casing had a choke and a discharge line to the tank hooked up to it to blow it down and was opened to the tank at 8:20 am with 1200 psi on it. By 9:00 am the pressure was 1100 psi, by 9:40 am the pressure was 500 psi and by 11:05 the well was still gassing at an estimated 50 to 100 psi. The pump truck pumped 100 bbl of water with 6% gel down to load the 8 5/8" casing by 12:30 pm. A total of 263 bbl of water was pumped down to load the well by 1:20 pm. The 8 5/8" casing had pressured back up to 75 psi by 1:40 pm and was building slowly. The job was to be continued the next morning with the arrival of more water to load the well with.
2753656 Primary Facility 2018-07-18 Plugging(Includes Plugged/Mined Through) Pending On this date the Well was loaded with water and the rig ran in the hole and shot equalizer holes in the 4 1/2" at a depth of 6036'. The rig then ran in the hole with a casing cutter, tagged the TOC inside the 4 1/2" casing at 6217' and shot with the casing cutter at 6156.5' at 12:06pm. The crew then began disassembling the well head in preparation to attempt pulling the 4 1/2" casing.
2753774 Primary Facility 2018-07-19 Plugging(Includes Plugged/Mined Through) Pending On this date the rig crew finished getting the well head ready and began tripping the 4 1/2" casing out of the hole. The casing was coming out very nicely at the time of this inspection. The forward plan is to continue tripping pipe out of the hole today and tomorrow. I am off tomorrow, Friday the 20th and have informed Site Supervisor Josh Williams that He can call me if he needs anything.
2754860 Primary Facility 2018-07-23 Plugging(Includes Plugged/Mined Through) Pending On this date, the Rig was able to trip pipe back inside the cut top of the 4 1/2" casing which was at 6156' and set down on the 2 3/8" tubing inside the 4 1/2" casing at 6207'. They then spotted 130 bbls of 6% gel for a total of 210 bbls. After spotting the gel, some gas showed on the 8 5/8" casing. The well was then shut in and built to 20 psi before reloading the hole from the surface for the night. The forward plan is to cement from 6207' to 5707' which would leave gel up to 2200'. The logging truck is to be on site at approximately 2:00 pm to rig up and log the 8 5/8" casing from the 2101' casing seat to surface.

Operator rep Josh Williams present during inspection. E and S plan on site. At time of inspection temporary BMP's near Cranberry Glade Run appear to be functioning as intended. No sediment is entering Cranberry Glade Run at this time however it appears maintenance may be required to ensure BMP's on upstream side of Cranberry Glade Run continue to function as intended (photo #1). This BMP (CFS) does not appear to be staked and may be at risk of failure. Department recommends operator continue to monitor and maintain these temporary BMP's. Mr. Williams states, in addition to operator E&S contractors, he inspects BMP's when he is on site. Mr. Williams also states he will contact E&S personnel today and relay Department's concerns. At time of inspection well appears to be plugged. Erected monument present with permit number permanently attached (photo #1). Plastic liner remains on site. Department collected soil conductivity readings at site. Vegetation down-gradient (south) of site continues to appear stressed/dead (photo #2 and 3). These areas of stressed/dead vegetation do not appear to be growing larger. Conductivity readings in these areas demonstrate some elevated levels compared to background readings. When standing on well pad facing south, the area to the left (east) of the rock channel exhibits elevated conductivity readings compared to background readings. This area appears to have been seeded and mulched but remains un-vegetated (photo #4). Department recommends operator follow up with Department regarding operator plans to ensure this area is remediated and restored per current regulations. Department contacted operator rep Corey Giles by phone to request update on site remediation. Mr. Giles states he was on site Monday (8-6-18) and collected soil samples in the 3 areas down-gradient of the well pad. Mr. Giles states those areas will be seeded to encourage growth. Mr. Giles states the area lacking vegetation to the east of the rock channel was previously excavated, seeded, and mulched and will be re-evaluated due to lack of vegetation. Mr. Giles states the rock channel was installed to avoid accelerated erosion due to the natural channelization of water in that location. Department recommends operator remove rock channel when restoring site. Department requests operator submit documentation showing grid system plan operator used to assess impacted areas of site. Based on submitted documentation, it appears operator intends to seed and mulch impacted areas down gradient of well pad. Department collected conductivity readings in these areas on 8-9-18. These readings were elevated and appear to indicate brine-impacted soil. Department recommends operator follow DEP Document #800-5000-001: Addressing Spills and Releases at Oil and Gas Well Sites or Access Roads, and excavate soil that appears impacted by the 6-14-28 release.
