This inspection documents on-site interview of operator representatives on 6-15-18. According to Josh Williams, gas and brine was released from the well at approximately 12:15 on 6-14-18. Mr. Williams states that the equipment was shut down at that time but the release continued. Mr. Williams also states that when the gas/brine was released, brine was sprayed over the equipment/site and brine was spread by the wind away from the well head. In addition, Mr. Williams stated that when the plugging equipment was shut down, the well cellar overflowed. Mr. Williams stated no fluids from the well cellar left the site; a small amount of fluid overflowed one corner of the cellar.
